This is why multidisciplinary collaboration is so important. We are not just managing the visual impairment. We are helping the person so that they can cope better with their activities of daily living

Visual Impairment Linked with Certain Neuropsychiatric Symptoms of Dementia
These included hallucinations, depression, agitation, psychosis and mania, but some of this relationship may be explained by age, comorbidities and other factors.

In terms of ocular care, the study authors believe that that, “given the increasing prevalence of sight-threatening eye disease such as cataract, diabetic retinopathy and macular degeneration among the elderly population worldwide, these findings highlight the importance of early and ongoing ophthalmologic care for older adults beyond correcting refractive error because effective treatments such as surgery, laser photocoagulation and intravitreal injection may halt or slow the progression of vision loss in these disease entities.”

Research confirms sustained improvements from vision therapy.

Long-Term Benefits of VT for Convergence Insufficiency Sustained in Children
One-year follow-up shows improvements in NPC and positive fusional vergence after 16-week vergence/accommodative therapy were maintained.

“There were similar percentages of participants classified as ‘normal,’ ‘normal and/or improved’ and ‘normal and improved’ based on near point of convergence and positive fusional vergence at the one-year visit compared with the 16-week primary outcome visit," the researchers wrote in their study.

Red light therapy

Study Finds Red Light Therapy Cuts Ocular Elongation Rate and SER Increase
Fourteen months of TID application saved 0.28mm of AL growth and -0.64D of refractive progression in treated subjects.

Clinicians outside the US are already using low-level red light therapy to slow ocular growth in pediatric myopia cases, as this treatment has been shown to substantially reduce rates of myopia progression. The treatment is not yet available here, but US optometrists and ophthalmologists have been following the treatment’s ups and downs with interest in anticipation of potential future adoption in their practices. The treatment came in for some recent criticism after safety issues were reported, but has generally shown positive results.

Hot off the press! Dr Yap Tiong Peng (IGARD, Singapore) and Dr Cathy Stern (Boston, USA) talks about how vision therapy teaches the brain to use both eyes together. They shared about how double vision can be treated. Both Dr Yap and Dr Stern are Fellows in Optometric Vision Development and Rehabilitation (College of Optometrists in Vision Development, USA).

In the photo is a child who was successfully treatment for strabismus with vision therapy at IGARD. Established since 2004, IGARD Centre is fully accredited in vision therapy and neurodevelopmental / behavioural optometry, including amblyopia / strabismus treatment without surgery www.igard.com.sg

Hot off the press! “Vision Therapy teaches the brain to use both eyes together”, written by Dr Yap Tiong Peng (IGARD Singapore) and Dr Cathy Stern (Boston, USA) to address the recent concerns amongst community optometrists due to the spike in cases of double vision as reported by the Singapore National Eye Centre. As reported by the Straits Times, the increase in cases of double vision in Singapore usually does not require surgery.

Now you know. Therapists and doctors need continuing education too!

Here’s the reason if you’re wondering why it’s been so difficult to get an appointment lately — Dr Yap Tiong Peng recently completed his continuing education with Harvard Medical School in Neuro Rehabilitation 2024 (May 15-17). Although Dr Yap has been working on neuro visual rehabilitation and vision therapy for many years, there’s still a lot to learn as the field of neurorehabilitation entails the understanding of neurology and its interdisciplinary collaboration with many healthcare professions. This year, Dr Yap published a book chapter on the neuro optometric approach in managing hemiplegia. In addition there will soon be another book chapter by Dr Yap on the “Advances in Managing Oculomotor Dysfunction in Concussion” under the book series on “Current Practices in Ophthalmology”. Essentially, oculomotor dysfunction has to do with visual tracking skills which can contribute to symptoms, such as headaches, eyestrain and dizziness, and can hinder activities such as reading. Also in the area of traumatic brain injuries, Dr Yap is currently on the writing committee on concussion with the American Academy of Optometry - Cochrane Eye and Vision (AAO-CEV). Watch this space for more updates.

Case Series: Neuroplasticity and Vision Therapy in Adults with Unilateral Small-Angle Esotropia | Published in Clinical Insights in Eyecare 30/04/2024

Vision Therapy Success!!!! Adults with long-standing constant strabismus may have vision goals related to binocularity. Here’s 3 interesting case reports of adults with unilateral esotropia who achieved random dot stereopsis with vision therapy. These three cases highlight the success that can be attained in motivated adult patients when visual therapy is used. This suggests neuroplasticity continues even through adult life. Vision therapy can facilitate this process in adult patients with longstanding strabismus to achieve stereopsis.

Refractive Error Change and Overminus Lens Therapy for Childhood Intermittent Exotropia 28/03/2024

Will myopia worsen using spectacles “overminus” lens treatment? (Dr Yap has the answer!)

Spectacles are often prescribed as part of the treatment plans concerning vision problems, such as strabismus and amblyopia. It is a very important consideration for patients undergoing vision therapy, so that an optimal spectacle prescription is used during the course of the treatment. At IGARD, vision therapy is usually recommended to children and adults with strabismus, including intermittent exotropia. While there are various treatment approaches, it is important to carefully consider the spectacle prescription with a doctor who is specialised in vision therapy. As proven from this recent research, childhood myopia tends to worsen with “overminus” lens treatment. It is important to clarify that this treatment approach did not involve vision therapy. However, the myopic shift became similar between overminus and non-overminus spectacles following treatment weaning/cessation, as reported in the research article. At IGARD, we focus on evidence-based approaches in our vision therapy and treatments. The research article can be accessed from this website:

Refractive Error Change and Overminus Lens Therapy for Childhood Intermittent Exotropia This extension of the Trial of Overminus Spectacle Therapy for Intermittent Exotropia compares refractive error change over 3 years in children with intermittent exotropia originally treated with overminus vs nonoverminus spectacles.

Dr Yap has also published on the topics on “Dyslexia and Vision”, and more recently on the topics on “Electrodiagnosis and Psychophysics of Amblyopia”, “Treatment of Oculomotor Dysfunction in Concussion”, and “Children Visual Development”. He has presented at many international conferences, and completed more than 100 peer reviews with scientific and ophthalmic journals worldwide. He is currently on the Editorial Review Board of the Journal of Optometry and Visual Performance. Concussions in young children: symptoms still present three months later 23/03/2024

Children may not be complaining but the symptoms may last for months after a concussion. This is a finding from a recent research which examined postconcussive syndrome using developmentally appropriate measures. Some of these symptoms may be related to vision problems too, which is why it is important to get the children checked by an optometrist who is able to assess the children’s visual functions using age-appropriate tests.

Concussions in young children: symptoms still present three months later A study led by Sainte-Justine hospital researcher Miriam Beauchamp has found long-term symptoms in children aged six and under who suffered a concussion.
