Just out in the Journal of Traumatic Stress: an investigation of the association between PTSD severity and distress tolerance in traumatic stress treatment.

Distress tolerance, or the perceived ability to tolerate negative emotional states, is often associated with posttraumatic stress disorder (PTSD) such that higher distress tolerance is generally associated with less severe PTSD symptom levels. As distress tolerance is often considered a risk and maintenance factor in distress disorders, examining the association between changes in distress tolerance and changes in PTSD symptoms may have clinical relevance.

The present study examined the associations between PTSD symptom severity and distress tolerance across three assessment points over 12 weeks among 212 patients receiving outpatient psychotherapy services.

The study found that distress tolerance did not predict later PTSD symptom severity. However, when examining the relationship between distress tolerance and four PTSD symptom clusters over time, it was clear that only the PTSD symptom clusters predicted changes in distress tolerance, not the other way around. The findings suggest that while distress tolerance is linked to changes in PTSD symptoms, it plays a significant role in symptom remission during treatment rather than influencing symptom severity directly.

How do trauma survivors experience changes after therapy? ''It made me feel more alive''

Posttraumatic stress disorder (PTSD) often severely impacts quality of life. While trauma-focused therapies (TFTs) like prolonged exposure (PE) and cognitive processing therapy (CPT) are known to improve this, how do trauma survivors actually experience these changes?

A new qualitative analysis, published in brief report form in the Journal of Traumatic Stress, explored this question with a national sample of 60 veterans who completed TFT at U.S. Department of Veterans Affairs facilities. Six key themes emerged, revealing positive shifts in their quality of life:

1. Full participation in social activities 🎉
2. Greater emotional intimacy in relationships ❤️
3. Improvements in parenting 👨‍👩‍👧‍👦
4. Expanded engagement in hobbies and community 🎨
5. Increased occupational commitment and confidence 💼
6. More joy in life 🌟

These findings underscore the transformative impact of TFTs on the lives of trauma survivors, offering rich, qualitative insights that complement existing quantitative data.

New research in the Journal of Traumatic Stress examines emotion dysregulation in refugees. 🧠

The refugee experience is typically characterized by exposure to numerous premigration traumatic events and postmigration stress in the resettlement environment. Refugees’ experiences can lead to elevated rates of psychopathology, including posttraumatic stress disorder (PTSD) and depression. Emotion regulation is a key mechanism contributing to mental health outcomes among refugees.

This study found that emotion dysregulation is associated with PTSD and depression severity, particularly in older refugees. Higher emotion dysregulation also predicted lower social engagement and greater functional impairment. These findings highlight the importance of age-specific interventions for refugee mental health and social adaptation.

📊 A new study in the Journal of Traumatic Stress evaluates the PTSD Checklist for DSM-5 (PCL-5) in women experiencing intimate partner violence (IPV).

Recent national estimates suggest that 1 in 2 women will experience intimate partner violence (IPV) in their lifetime. Given the high prevalence of posttraumatic stress disorder (PTSD) among women experiencing IPV, it is necessary to consider whether PTSD assessments function adequately for this population.

This study aimed to evaluate the psychometric properties of the past-month version of the PTSD Checklist for DSM-5 (PCL-5), one of the most widely used self-report measures of PTSD symptoms, in a sample of community women experiencing IPV.
The PCL-5 showed good internal consistency but poor 30-day stability for this population. A lower cutoff score of 21 maximized sensitivity and specificity, compared to typical cutoffs of 31-33. This suggests the PCL-5 may underestimate PTSD prevalence among women experiencing IPV. Findings indicate the need for careful consideration when using the PCL-5 with IPV survivors.

This work aims to improve assessment and treatment approaches in traumatic stress studies.

⭐ Tipping the scales for global mental health: A summary of the Presidential Panel at the 39th Annual Meeting of the International Society for Traumatic Stress Studies ⭐

In the past decade, there has been an increasing focus within scientific research on how to assist people affected by the negative consequences of trauma and crises around the globe. As many countries struggle with a lack of resources to deliver mental health interventions, scalable strategies have been developed to help more people in need. These scalable strategies were the theme of the 39th annual meeting of the International Society for Traumatic Stress Studies (ISTSS).

The presidential panel, chaired by Marit Sijbrandij during her ISTSS presidency, brought together a group of experts in the field of scalable interventions: Kenneth Carswell, Syed Usman Hamdani, Judy Bass, and Eirini Karyotaki. The panel highlighted the current state of the evidence on scalable interventions for adults and children and outlined important next steps for research and implementation. These recommendations include further improving the availability of, and evidence for, scalable interventions through increased training and sustained funding; conducting more studies in underrepresented samples, such as children and adolescents; and promoting open access availability of research findings worldwide.

In this paper, an overview of the topics discussed in the panel as well as the key takeaways - including the effectiveness of interventions like EASE for adolescents and SH+ for refugees - are provided.

✨ Just published in the Journal of Traumatic Stress: ''Bidirectional associations between prolonged griefsymptoms and depressive, anxiety, and posttraumatic stresssymptoms: A systematic review.'' ✨

Prolonged grief symptoms frequently co-occur with symptoms of depression, posttraumatic stress, and anxiety; however, little is known about how prolonged grief symptoms temporally relate to symptoms of neighboring stress-related and affective disorders. Clarifying such associations can help elucidate which symptoms to prioritize during treatment for distressed bereaved adults.

This new systematic review examines the relationship between prolonged grief and other mental health symptoms. Analyzing 8 studies with 2,914 participants, the review finds that prolonged grief often predicts later depression and PTSD symptoms. While the relationship appears bidirectional, it's complex. Early severe grief may exacerbate other symptoms.

A new State of the Science in the Journal of Traumatic Stress: Treatment of comorbid posttraumatic stress disorder and substance use disorders!

This state of the science review examines treatment for comorbid PTSD and substance use disorders (SUDs). PTSD and SUDs co-occur at high rates, with research showing that up to nearly 60% of individuals with PTSD also suffer from an alcohol and/or drug use disorder. PTSD/SUD is complex; associated with adverse health, social, and economic outcomes; and can be challenging to treat. Over the past decade, the landscape of treatment research addressing PTSD/SUD has significantly expanded.

Ongoing efforts aimed at developing and evaluating novel treatments for PTSD/SUD, encompassing both psychotherapy and pharmacotherapy approaches, are steadily advancing. Rigorously conducted clinical trials demonstrate that individual, manualized, trauma-focused treatments are the most efficacious psychotherapies to use among individuals with PTSD/SUD. Importantly, abstinence is not required to start PTSD treatment. No medications have been established for this comorbidity yet.
However, promising research on integrated therapies and tech-based enhancements is ongoing.

The Evidence-based Practice Center (EPC) Program at the Agency for Healthcare Research and Quality (AHRQ) has posted the draft report for the systematic review on Trauma Informed Care (TIC). The purpose of the review is to examine how TIC and its components are defined and operationalized, and to examine the state of the evidence on effectiveness and potential harms of TIC approaches, frameworks, models, and components.

The draft report describes the findings from the evidence review. They are inviting the public, including professionals working in the psychotrauma field, to provide feedback on the draft. New Study Casts Doubt on CBD's Anxiolytic Effects for Trauma Victims:

A recently published clinical trial in the Journal of Traumatic Stress found that CBD may not effectively reduce trauma-induced anxiety, distress, and physiological arousal. The double-blind, placebo-controlled study evaluated the acute and repeated effects of a 300mg daily CBD dose in 42 trauma-exposed individuals. Participants underwent an idiographic trauma script challenge to elicit stress responses.

Contrary to claims of CBD's anxiolytic properties, the results showed no significant differences between CBD and placebo in reducing anxiety, distress, or heart rate reactions to the trauma script. This was true for both single-dose and 1-week treatment conditions. While more research is needed, these findings suggest CBD may not be the panacea some have touted for trauma-related symptoms. The authors note the small sample size as a limitation, but conclude CBD does not appear to robustly attenuate trauma-relevant emotional reactivity.

This adds to the growing body of mixed evidence on CBD's clinical efficacy. More large-scale, rigorous trials are necessary to clarify CBD's therapeutic potential, especially for mental health conditions like PTSD.

New on the Trauma Blog: Adapting Trauma-Focused Psychotherapies to Expand the Reach of PTSD Care

Mental health treatment for posttraumatic stress disorder (PTSD) in general, and trauma-focused treatment (TFT) in particular, is traditionally delivered by trained expert mental health clinicians, such as clinical psychologists and licensed clinical social workers. TFT, on account of being more challenging to clients than non-trauma-focused treatment (to say nothing of the misconceptions about its safety and tolerability), has been especially restricted to the purview of expert clinicians. However, research over the past decade has begun to challenge some of our assumptions about how clients can effectively participate in TFT.

One assumption is that TFT must be delivered by a provider (i.e., face-to-face or telehealth). However, a recent study by McLean et al (2024) represents an attempt to simultaneously challenge assumptions about how TFT can be delivered and who can support trauma-focused internet-based interventions (IBIs).

1. What are the potential benefits (and possible pitfalls) that should be considered in the development of IBI’s for PTSD?
2. What unique patient and provider barriers might be important to consider for online adapted TFT for PTSD?

Fi****ms have also become the leading cause of death among children and adolescents in the United States in recent years.
