When Psoas minor isn’t minor...

I recently had the honor of working with an amazing midwife during an ECV. As she was gently helping baby rotate, she ran into a “ledge” in the upper abdomen that baby just couldn’t get past. I wasn’t exactly sure at the time what anatomical strut was preventing rotation past that point but I could feel a band just under the head. As I gently worked the fascia in the area, the band softened and baby’s head rotated as soon as it released. It was a beautiful collaboration!

A few days later, I get a text from the woman asking what could be wrong since she started having pain in her anterior thigh after the ECV. Hmmm could she have a Psoas minor?

On the next visit, palpation definitely indicated the presence of P minor on the left. Baby was also kicking on the left indicating an RO position. We discussed the possibility that baby may be born posterior as she might be unable to rotate around Psoas minor. Sure enough, baby was born straight up OP a few days later.

Birth has always held a sense of mystery.

The baby was seen as just something on the inside and didn’t seem to play much of a part in the process.

Thank goodness that paradigm is shifting. But what can we see from the outside?

What if there was a way to feel what was going on inside from the outside?

What if there was a way to feel that baby wasn’t in just the right position during labor?

What if the mother’s body held those clues?

Guess what? The sacrum of the laboring person can help reveal what is going on with the baby’s position on the inside.

The baby affects the position and motion of the sacrum during labor.

The understanding of the birth process must change in our society. The status quo maintains that babies come when babies come and there isn’t anything that can be done from the outside to change that. The baby isn’t seen from the outside and stays hidden during the birth process. The baby is being birthed and is not seen as an integral part of the process.

However, as the paradigm shifts, we see that the baby’s position in utero is an integral part of the birth process. Baby’s position can dictate if the mother will easily go into labor or not. The position, movements, and ease of the baby can start and stop contractions in some cases.

I recently witnessed this process after my clients waters began to flow yet no contractions were present. Her body was recognizing that her baby was not in a good place for birth.

There was pain in places there “shouldn’t be”.

The baby was not moving as easily but her stats were good.

Baby was stuck in a less than optimal position. Pain points suggested a nuchal hand.

Baby’s position was preventing contractions and her mother’s innate intelligence recognized this and honored that her baby wasn’t ready.

We worked to change the biomechanics of the mother...to create space...to create more ease and baby utilized this opportunity to find a more comfortable position.

Contractions started. Labor started as baby shifted into a better place.

Mother and baby worked as a team together in an intimate dance responding to each other through the night.

The process of birth must be reborn as a team process taking into account both the birthing person and the person being birthed.

Their biomechanics work together when balance and ease are present.
