✨REAL LIFE STORY✨
Carolyn’s story- Lived Experience of Episodic LBP x 18 years.
My first and worst episode of LBP started 2/12 after my daughter’s birth in 1990. I was sitting on the floor reading a story to my two-year-old when my back spasmed. It was significant! I was “locked up”.
At this point in my career, I understand that with a levator ani avulsion and two pregnancies 18 months apart, this wasn’t surprising (no, I did not have pelvic floor physio 34 years ago) and I hadn’t strengthened my core sufficiently (or my pelvic floor).
The worst thing was that I had a bath within the hour of my back spasming and my 6-foot frame could not get out of the 5-foot bathtub after a 30-minute bath. My sister and husband managed to help me out after a long struggle and a threat to call the paramedics at the small hospital I worked at to get me out of the bathtub. I managed but it was quite frightening!
34 years ago, we believed in the “permanence” of disc pain…. I guarded it for 18 years and did preventative sloppy pushups, used a lumbar roll, and took good “care” of my spine. Despite that, I had at least 1-2 episodes/year, for 18 years.
Then I took a course with Debbie Patterson in 2008. From her course, I understood that “fear” plays a role in sensitizing the nervous system and created a protective, pain response on a regular basis.
I challenged my confirmation bias’ in 2008 and understood that my low back “injury” healed many, many years ago. Once I stopped doing protective sloppy pushups, and threw my lumbar roll away, I never had another episode of back pain again.
That was 16 years ago now!🤯

Exhale/contract is one way not the only way.
What if Inhale/contract helped the coordination ? What if you could *gasp* get a better contraction on inhale? I’ve seen it happen.
And usually the person is so shocked because they’ve been told the opposite. Great- you are good that way. Now you get to be good at many ways. Train the pelvic floor like you expect it to adapt because it does and you may just see your performance improve.
Improvement in 3 sessions . She’s been dealing with these issues for about 2 years and finally sought help.
FYI- Physical therapy doesn’t have to be 3x a week for 6 weeks in order to see improvement in symptoms.
Patellofemoral pain
2nd most common knee pain in adolescents.
Affects 16.5% of runners and women are 2 x more likely to have it than men.

The common narrative around Patellofemoral pain has focused on structure alone.
This leads to people being sidelined from activity until pain subsides.
A quick google of Patellofemoral pain syndrome advises - rest and avoid activities that cause pain or don’t run until the pain is gone. 😢

So structure that needs to be fixed + avoidance advice = no wonder people are afraid to exercise or load the knee joint.

But
Loading the knee is valuable and can be done in ways that aren’t as symptomatic . Like changing pace (increasing step rate), managing load intensity or overall volume, adding variability to the load, building one’s capacity. This can all be done according to the persons unique goals.

Stop giving core exercises to fix back pain and start building resilience which may include core exercises.
I have absolutely had people ask me to help them build more core strength.
I’m ok with that. Meet people where they are at. The exercises are then challenging at their current capacity. And sometimes their current capacity is rather strong!

Quad injury -
Most of the time my schtick is helping you return to what you love with persistent pain.
This is an acute injury. This is when you rest.
1. Definite mechanism of injury
2. Notable Swelling
3. Reduce function , range, and capacity.
4. Pain with activity
5. X ray came back normal.
6. Working around Soft tissue injuries- respect the healing timeframe.
-Continue working the rest of the body for mental and physical health. -Gradually progress the load and capacity of the injured body part. -Always test and retest for capability. -Aid resiliency and variability to reduce further injury risk.
-Last but not least make sure the capacity meets sports specific demands.
