Why do IUDs get a bad rap? 🤷🏻♀️
Often patients are hesitant because of safety, efficacy, and potential side effects. (also videos on tiktok)
And the horror stories. (Be assured there are success stories as well.)
→ People fear it will migrate through the uterine wall. The risk of uterine perforation is 1/1000. Placing the device with ultrasound guidance can decrease your risk of this complication.

→ People are afraid they will have pain with the placement of the IUD, with in*******se, and/or with removal. There is no question that this can be a painful procedure due to the need to dilate the cervix. Premedication prior to the procedure can help. ASK YOUR DOCTOR TO PROVIDE THIS! If you have attempted placement but were unsuccessful due to pain, light IV sedation is even an option.

→ These are all valid concerns to process with your provider. There are ways to mitigate potential pain on insertion of IUDs, and removal is far easier than placement.
The numbers speak for themselves - IUDs are more than 99% effective at preventing pregnancy. 👊🏻

An IUD can stay in place for 5-10 years, depending on the type, and can be taken out at any time (by your healthcare provider).

Also, did you know they are used for other things than contraception? IUDs are frequently used to control bleeding in women as they move through the menopausal transition. Progesterone in the arms of the IUD is secreted daily to thin out the lining of the uterus (endometrium). Wanna know more about this? Stay tuned.

Rose all day, wine flies when you are having fun, why limit happy to an hour? Our society lives for a reason to celebrate with a cocktail and it begins to feel like self-care within the first few sips. And, the more stressed and busier we are, the more we desire the self-care. In fact, more than 60% of people drink solely to reduce stress.
Busy, stressed women reach for a cocktail because alcohol can make us feel more relaxed and more confident. This reduction is called stress-response dampening (SRD) . Alcohol helps us escape the negative feelings in our life. SRD can be a vicious cycle, alcohol consumption reduces our emotional and physiological response to stressors, which in turn motivates us to drink more. It can even help our emotional response to trauma.
As we drink we feel calmer but this may only be a temporary fix. Alcohol is a depressant, so while it is calming & soothing (it may hit some of those same receptors as drugs like Xanax or Va**um) it can cause rebound anxiety. The next time you have a lengthy day of drinking, take note of how you are feeling more irritable and anxious in the days following.

📆 Colon Cancer Awareness Month 📆
We're talking to our ladies over 45! It's time to consider getting screened!
Colon cancer is the 3rd most commonly occurring cancer in both men and women—of all races.
Getting screened regularly can increase your chances of finding, treating, and potentially surviving colon cancer. Because the risk of colon cancer increases with age, you should get screened, even if you have been screened in the past.
Cologuard is intended to screen adults 45 and older who are at average risk for colorectal cancer by detecting certain DNA markers and blood in the stool.
We can prescribe Cologuard here at Charleston House. It's a non-invasive DNA test that you can use in the comfort of your home 🙌 🏠

So, what is the difference between Ozempic, Weegovy, Rybelsus, & Semaglutide?

Ozempic, the brand name for Semaglutide, was first studied in diabetics for sugar control, not weight loss. However, patients were losing weight quickly, an unexpected side effect. No other diabetic drugs have resulted in such weight loss. So smart folks at the drug company then rebranded the same drug with the indication for weight loss and named it Weegovy. Both Ozempc and Weegovy are Smaglutide. Weegovy is the FDA-approved form of Semaglutide for weight loss, and Rybelsus is the oral form of Semaglutide, which is also FDA-approved for diabetes.

Semaglutide, whether it be Ozempic or Weegovy, has been safely used in my practice for years before this new movement. I started prescribing these medications right after COVID after doing some initial training in obesity medicine. You will find very few physicians who do not fully support using the medications. Unfortunately, the media is HEAVILY focused on the downsides of using Semaglutide.

Menopause is having a moment. You may have seen the cheeky ad during the Super Bowl on hot flashes that brought menopause to the stage before millions of women. A few weeks ago, a wonderful article was released in the by that I thought we should discuss.
I left my busy OBGYN practice in 2019, knowing there was a deficit in healthcare for mid-life women. Determined to fill that gap, Charleston House was created. I engrossed myself in menopausal medicine & became a certified North American Menopause Society practitioner, meeting brilliant folks forging a path for both women & doctors like myself. I invited my OBGYN colleagues to join me in my involvement in NAMS. Yet, the most trained professionals in women's health were unaware that the entity existed. Couple the lack of awareness among practitioners with a paucity of research & bad press on hormone therapy, & women have been left empty-handed. This article beautifully depicts the uphill climb for women to find safe & adequate treatment & the negative press surrounding hormone therapy.
Fortunately, a lot has changed over the last 4 years, & the menopause industry has since exploded. My insta feed is a continuous stream of influencers, telemed platforms, & treatments. There are lotions & potions galore, & celebrities touting their benefit. While that is exciting, it can be fraught with land mines for the vulnerable consumer. I say be cautious. Know that “natural” does not always mean safe. There is no “quick fix” for menopause symptoms.
Today, I'm thrilled that more of us are interested in caring for mid-life women. We need to share our resources and learn from experts & educated writers like who are not influenced by the onslaught of PR attached to this subject to spread the good word.

This month and every month, we recognize that cardiovascular disease (CVD) is the leading cause of death in both pre-menopausal and post-menopausal women.
Here are some surprising facts:
- CVD kills more women than all forms of cancer combined
- One death occurs every 80 seconds
- 64% of women who die from sudden heart disease have no symptoms
- Women are more likely to dismiss their symptoms than men.
- Lesser known risk factors for development of CVD in women:
PreEclampsia/Hypertension in pregnancy
Gestational Diabetes
Premature Ovarian Insufficiency (POI) Menopause before
40 yo
- Age matters: Risks begin to increase at age 40
- Menopause status matters: Menopausal women develop an increase in blood pressure and cholesterol more than a non-menopausal woman the same age.
- Family history matters, even when you are considered to be "low risk" by other factors.
Action: SCREEN every woman at every visit.

Despite countless rejection letters, being voted in by a student body of men as a practical joke because they thought she couldn’t keep up, she paved the way and serves as an inspiration still today.

Ever wonder what we do here at Charleston House? In short, we do things very differently than you might expect from your OBGYN visit.
The care we provide is uniquely prescribed & driven by the needs of the woman sitting in front of me. Each & every interaction with a woman is different.
Our Approach: Our women start with an hour-long consultation BEFORE their physical exam. Women shouldn't have to undress to talk to their doctor. Then we set out a plan to meet those goals & address concerns. Follow-ups are scheduled to discuss labs & treatments, plus additional follow-ups every three months on any issues we are working on, often by a virtual platform. We have an ongoing dialogue about your health.
At the initial appointment, you are given a direct line you can use to text nurses. You are given a direct number you can text our scheduler. And you are given my personal cell number for immediate use.
We walk our women through personal tragedies & we hold them up in times of uncertainty. It may sound lofty, but it’s how we make the best use of their trust & our relationship.

January should be called National HPV Month. Without HPV, cervical cancer would have little game. You can't talk about cervical cancer without first giving credit to the main culprit.
Should you be worried about cervical cancer? Only if you haven't had a normal pap smear in a few years, AND if you have had new partners and here's why:
HPV is EVERYWHERE, and it's not going anywhere for a long while - maybe never. It is estimated that more than 80% of sexually active women can be exposed to HPV. And since Pap smear screening guidelines are so confusing, we need to understand how to prevent both HPV and cervical cancer to the best of our ability.
Her are a few nuggets to remember:
HPV is sexually transmitted.
New partner, new exposure.
New exposure, new strain.
New strain, pap smear can be abnormal. (in fact, checking a paps smear in close proximity to a new partner increases your chance of having an abnormal pap smear.)

Do I need a new pap smear every time I have a new partner? No
Can my pap smear be normal after new partner? Yes

More than a bank holiday or a day off from school, today is a day of reflection and remembrance of Dr. King's plight for equality, justice, peace and love for others. His words spoken over 50 years ago still ring true, as we move deeper into 2023 ask yourself, how will you be serving others?
We are often asked why a woman would need a concierge gynecologist. Our answer: Women need comprehensive care that includes access and time spent with a doctor. This type of relationship extends beyond what we women have come to accept at our "annual visit." Breast exam, pap smear, and a quick "you're fine" should not be the accepted care path for a woman. More care looks like this: An in-depth relationship with your doctor, time spent discussing chronic disease and cancer prevention, attacking quality of life issues like sleep, mental health and weight management AND a road map to living a longer and healthier life.
