Bilateral ni**le-sparing mastectomies and staged tissue expander implant reconstruction.

Years after chemotherapy and radiation treatment for Hodgkin’s lymphoma, this patient felt a breast lump on self exam. Diagnosed with breast cancer she chose bilateral mastectomies and reconstruction.

Without ideal flap options in a thin woman, tissue expander implant reconstruction can provide a fuller volume breast reconstruction when desired.

With ni**le-sparing and nerve-sparing mastectomy approaches today, hidden scars can frequently be placed below the breast at the first stage. Expansion in the office allows patients to decide when they are happy with their breast size and ready for stage two implant exchange, done as an ambulatory procedure.

This is my mom.

Diagnosed with breast cancer 15 years ago, she is a survivor.

During her career as a psychologist, she helped improve the lives of countless individuals.

Now an accomplished poet, she continues to help and inspire others.

Written after completing her treatment for breast cancer, here is her beautiful poem about survival and hope.


“SURVIVAL SEQUENCE”

Diagnosis

Struck by lightening –
my life sharply divided
into a before and an after
and the marks of the incision hurt.

Foe

A minuscule tumor
turns light into darkness,
like a small ominous cloud
threatening to cover the sun.

Probabilities

Compelled to speculate:
How many more birthdays?
Will I be there when…?
What are the odds?

Battleground

My body a combat zone
with parts missing in action,
the scars – inscribed memorials
to all that was lost.

Surgery and Beyond

On the operating table,
I die a little. But waking up,
I greet the blue sky
with new clarify.

Healing

Life is pulling me back –
whispering in its alluring voices:
Come back… Come!

Meet my patient Jen!

Jen’s energy is contagious as she focuses on raising awareness of breast cancer.

She is an accomplished musician and singer, and certainly the leader of the NYBRA Plastic Surgery band where she performs at all of our Uplifting Events, which have help raise funds for the American Cancer Society, the Adelphi Breast Cancer Support Program, the Plastic Surgery Foundation, and Breast Cancer Comfort Care.

Jen is a shining example of how creativity is a wonderful outlet for stress relief, healing and awareness. I dare you to watch the video of her singing and not smile. it's impossible!

Meet LuAnn, breast cancer survivor, and inspiration to those around her.⠀

At NYBRA Plastic Surgery our patients aren't just numbers. They are women, mothers, sisters, daughters and wives. We don't just help rebuild their breasts, we help rebuild their self-esteem and their lives. We are always amazed by how many women are touched by breast cancer and equally amazed by how they turn their cancer into incredible gifts to the world. ⠀

My patients are always inspiring me in a multitude of ways. They are the reason I do what I do. ⠀

LuAnn is my patient and had mastectomies with breast reconstruction 9 years ago. Here I am with LuAnn at the 2019 Jones Beach Making Strides Against Breast Cancer Walk, helping raise awareness and funds for the American Cancer Society! To walk beside her and other patients in support of a cure for breast cancer is always an honor. ⠀

Today is the 10th Anniversary of the release of the Song ‘We Believe’ by my patient Kate Corrigan. Kate is incredibly talented and has used her talent to raise awareness and joy by performing the song at fundraising events for The American Cancer Society and at our first ever uplifting event in April 2010 (swipe to see the video!)⠀

October is just around the corner. Being that it is Breast Cancer Awareness Month, throughout the month, I will be posting about many of my patients who have turned their struggles into a mission of empowerment and strength.⠀

Meet Rachel. BRCA positive. Patient advocate.

“When I was diagnosed, I didn’t want my kids to worry, I didn’t want my patients to know, I kept it quiet in the community.”

That’s how Rachel, an Orthodox Jewish s*x therapist, described the immediate aftermath of learning that she had breast cancer on her 40th birthday.

Over 15 years later, she sees things differently. “I speak about my journey publicly now. There’s no shame in this. You don’t have to hide it, and you don’t have to suffer alone.”

Rachel had a lumpectomy, chemotherapy, radiation, and Tamoxifen. “When I was nearing the end of the Tamoxifen, my oncologist suggested the BRCA test, which had not been done because I had no family history anywhere.”

The test came back positive and Rachel entered the BRCA positive world. She had bilateral risk-reducing mastectomies with DIEP flap reconstruction. But surgery was not her biggest hurdle.

“I was not traumatized by the surgery, in fact I really liked the breasts that I got, but then it became this whole world of being BRCA positive.”

Rachel has since offered her time to help support others. In the past, she has worked with Sharsheret, a national non-profit organization supporting women at increased genetic risk of breast cancer. As part of the Sharsheret family, Rachel has been a “link” helping other women through their diagnosis, treatment, and recovery.

In her words, “I share a lot, they know what I do, and things would come up.” Now Rachel formally presents on post-reconstruction s*xuality and intimacy to patient and partner support groups at NYBRA Plastic Surgery.

Rachel’s experience has inspired her to do related work within her community, where diseases like breast cancer still bear stigma. She’s working to help others see trauma like hers as an opportunity to learn.

Bilateral ni**le sparing mastectomies and today. We’re still catching up, doing breast reconstruction for women diagnosed early during the Covid crisis. Not enough time to take care of everyone during the regular work week!

Catching up.

At NYBRA Plastic Surgery we started off the year doing dozens of breast reconstruction procedures in January and February. Then we had to pause for nearly three months. Now we’re working hard to catch up.

One of the joys of my practice is collaborating with partners in the operating room. This week Dr. David Light and I were able to operate together doing DIEP flaps on a patient diagnosed in March. Such a relief for her to be able to safely get the care she needs.

Here I’m transferring the first under the microscope as Dr. Light is preparing the second flap.

Surgery family waiting room.

On a Friday afternoon with the operating rooms running at full capacity today, the surgical family waiting room would typically be packed. So strange to see it empty, knowing that families still can’t visit loved ones in the hospital.

At NYBRA Plastic Surgery we’re back to doing multiple DIEP flap and implant breast reconstructions this week, with three new reconstructions just today. As we are now taking care of women diagnosed at the beginning of the crisis, we are working to catch up and slowly get back to normal.

Grateful for another day bringing us closer to better times.

Donating plasma

Having fully recovered from , I have antibodies in my plasma that can attack and eliminate this virus. Since donated convalescent plasma is now being used as treatment for patients with potentially life-threatening COVID-19 infection, I’m hoping my plasma can help someone.

Operating Again

As surgeons, we can help again in the healing of our breast cancer patients.

Over the past two months, guidelines regarding surgical management of breast cancer patients rapidly evolved to the point where previously scheduled mastectomies and breast reconstruction procedures were delayed. Hospitals overwhelmed with COVID-19 patients could not safely accommodate breast cancer patients for surgery. While some started hormonal therapy, other patients began chemotherapy.

The stress for cancer patients and their caregivers has been immense.

Today is my first day back in the operating room. My patient is undergoing bilateral ni**le sparing mastectomies with immediate prepectoral implant breast reconstruction that was delayed due to the pandemic.

I’m so glad to be able to return to my role as surgeon and caregiver.
