What You Don't Know Will Kill You - Drakewell Drakewell's push for oilfield digitization helps operators, service, and rental companies leverage technology to run their businesses and share data more securely and effectively.

There are more details in the episode, but essentially: the machine worked fine if the operator set all the settings and then used it, but sometimes would NOT if the operator set a setting incorrectly on accident, and then went back and fixed that setting but typed too fast while doing so (all before actually starting the therapy).
