“Doc. I’ve had back surgery before. Do you really think you can help me?” This is a patient before and after a procedure in the low back. On the left if pre-surgery, and the right is about 20 minutes later when we were done. We were able to restore the height of the disc to almost normal in a way. This was done under twilight sedation and the patient went home after 30 minutes in the recovery room.

If I look extremely happy in this photo it’s because I am! I have been seeing Ken for 2.5 years and we have been trying countless things to help with his back pain and spinal stenosis. It had progressed to the point where he was walking hunched over and was using a cane and thinking about a walker! He couldn’t even walk down the hall in my office. We had multiple discussions about the minuteman procedure and he decided it was the right move for him. This is only THREE weeks after the procedure.

I caught him in the parking lot while walking the dog and I just had to video. We are so happy for you! No more cane, standing straight up and the pain he’s had for year is 99% gone! I finally realized he’s a little taller than me!

Introducing our newest employee, . She is a that has been trained in patient care. She can help ease your anxiety before, during, and after your visit. She as an asset to helping out patients in chronic pain and she LOVES a good pet and head scratch.

If you have had a lumbar fusion and continue to have pain in the lower back there is a good chance you have Sacroiliac Joint dysfunction. Dr Roufaiel can perform a 30 minute procedure to help reduce your pain twilight anesthesia.

“Treat patients how you would treat your own family” Most important bit of wisdom from my mother back when I started residency. Paintings behind me are painted by my mother in law. Family is a big part of my practice.

This patient could barely walk or get out of a chair. Failed ESIs was on pain meds. She had severe lateral recess and moderate central stenosis with a grade 1 spondylolisthesis at L4-5. This is her TWO weeks post minuteman at L4/5. She came dancing into my office and now feels like part of the family. She said her biggest regret about the procedure is that she doesn’t get to come back for another two months for a final post op check after physical therapy.

For those that do the procedure it’s easy to see the challenge that this case poses. This 87 y/o male used to walk 5 miles a day until 2 years ago. He had a TAVR 9 months ago and came to our office saying his heart was fixed but his had progressed to a point where he needs to sit down after 100 feet of walking. He was depressed that his heart was fixed via a minimally invasive procedure but his back was now a problem and he was not a candidate for more invasive surgery because of his advanced age. This was done under MAC sedation. Look at the space created between his SPs just with the pin! His leg pain was eliminated in recovery (no narcotics intraop). 20 years ago this man’s Aortic valve would not have been replaced at his age and his spine would not have been stabilized because procedures like the TAVR and the minuteman didn’t exist. We need to normalize and spread the word that therapies like this are available!

Here we have an 80 year old female patient with metastatic breast lesions at T8, T9 and T10 being treated by Dr. David Roufaiel of Advanced Pain Management and Wellness Center. He accessed the spine bilaterally and utilized Medtronic’s OsteoCool Radiofrequency Ablation followed by Kyphon balloon kyphoplasty to cement the vertebral bodies all through the same cannulas. The procedure took less than an hour and excellent results were achieved. It is phenomenal that we have industry leading technology at our hands. We have the opportunity to help people going through some of the most difficult times in their lives

Typical results 6 weeks post L4/5 minuteman procedure in a patient previously on opioids for 20 years. Gave up his motorcycle. Now the medications are almost gone and he’s getting back on the bike and literally dancing in my office!

What’s the difference between this procedure and any other one? This 88 y/o male has too many comorbidities to have even the slightest amount of sedation in the prone position. We tried one month ago with just medazolam IV and the patient nearly aspirated before we started. Thanks to the care of the HCA Florida JFK North Hospital team we decided to try again. I gave the patient epidural anesthesia and ample amounts of local anesthetic and the patient tolerated the 2 level bilateral procedure with no IV sedation whatsoever.

Sometimes we need to make changes to our regular protocols and never give up on trying to give our patients a better life.

This 79yr old sweet patient was told she was “too old” for back surgery. When epidurals failed and she came to see us we recommended the minuteman procedure. After a 30 minute procedure under sedation she woke up with zero pain in her leg for the first time in over a year and was ecstatic. 2 days post op she went for a 1/2 mile walk when she couldn’t even make it down her driveway before.
