Shades of pain: Understanding diversity in pain management

Language and cultural barriers can also limit providers' understanding of how patients are feeling pain, sometimes leading to misdiagnoses of its cause. "If a patient speaks a different language, the way they describe a shooting or burning or stabbing pain might be different from what providers are asking in English, even when using interpreters," said team member Shreya Louis, MD, a neurology resident. "When we as providers base our diagnoses so heavily on the description a patient gives, this language barrier plays a very large role."

About Corticobasal Degeneration (CBD)

Corticobasal Degeneration (CBD) is the rarest of the four atypical parkinsonism disorders. Depending on whom you ask, it’s classified as an atypical parkinsonism disorder, a non-Alzheimer’s dementia, or a type of frontotemporal degeneration. CBD is most closely similar to PSP in terms of symptoms and pathology. Many organizations that perform research on PSP also research CBD.

Because CBD is so rare, research has been very limited. Most studies of autopsy-confirmed disease only include a few dozen cases. It’s hard to be definitive with such small numbers.

The medical community now uses the term CBS or corticobasal syndrome to refer to a clinical diagnosis. CBD or corticobasal degeneration is now used for an autopsy-confirmed diagnosis.

About Progressive Supranuclear Palsy (PSP)

PSP is the second most common of the four atypical parkinsonism disorders. It is a rare disease. Depending on who you ask, PSP is classified as an atypical parkinsonism disorder, a non-Alzheimer’s dementia, and a type of frontotemporal degeneration.

PSP exists in two main variants: Richardson’s syndrome (PSP-RS) and PSP-parkinsonism (PSP-P). For the PSP-RS variant, primary symptoms are falls (usually backwards), dysarthria (slurred speech), and cognitive impairment. For the PSP-P variant, primary symptoms are tremor and a response to levodopa. For both variants, symptoms include bradykinesia (slow movement), and rigidity (stiffness)…in other words, parkinsonism.

Stanford University School of Medicine graduate, residency in Adult Neurology in 2016 and fellowship in Movement Disorders in 2017, both at Stanford University Medical Center. Dr Call sees patients with general neurological conditions and specialize in the care of a variety of movement disorders including Parkinson’s disease, atypical forms of parkinsonism, essential tremor, dystonia, Huntington’s disease, chorea, myoclonus, and functional movement disorders. Dr Call also helps care for patients who have undergone deep brain stimulation (DBS) surgery.
