Accessing technology may be a silver lining to this pandemic - especially for older Americans. As of 2020, more older adults than ever are using technology to access the world.

Local Elder Law firm is searching for a part-time Elder Care Coordinator (ECC) position. The ideal candidate is a semi-retired nurse or retired nurse looking to make a difference in someones life and must be located in Coquitt County, Lowndes County, Thomas County or one of its contiguous counties. The position is a contract position with part-time hours,10 to 20 a week with a flexible schedule. There is some travel involved and must have a dependable car. The candidate must have a background in nursing--RN or LPN required, geriatrc experience and/or home health experience is preferred and must be comfortable and professional with public speaking and desire the ability to market the practice with long term care facilities. Preference will be given to candidates with significant contacts in the long term care industry--nursing homes; assisted living; home health care, geriatric physicians and hospitals. Compensation depends on experience. Most importantly the candidate needs to have a desire to help others and enjoy working with seniors.

The ECC will develop long-term relationships with our clients and their families and oversee the coordination of the client’s health and long-term care, serve as an advocate, and empower the client and family with the knowledge one needs to make the right care decisions.

Additional duties include:

Conduct care assessments to uncover potential problems and find solutions.
Help locate and hire in-home help or other services.
Coordinate with medical and health care providers.
Review medical issues and offer referrals to other geriatric specialists to
provide appropriate care in a way that conserves financial resources.

Helps plan for the possibility of incapacity or disability.
Provide supports, guidance, and advocacy during a crisis.
Arrange for alternate housing, if needed.
Help to coordinate transfer and transportation of the client to or from their home, assisted care living facility, or nursing home.

Provide education and support.
Prepare and guide the client and family through the Medicaid application process, VA application process, guardianship, and other legal proceedings that impact the client’s care.

Report directly to, and work closely with the elder law attorney and staff, and others involved in client's situation including long-term care administrators, accountants, physicians and financial planners to ensure that the right professional is helping the client at the right time.

In addition to care coordination for the client, the person will also act as a community liaison, develop business for the firm, and speak on behalf of the firm along with the elder law attorney at seminars and community workshops on the holistic life care plan approach to elder law.

Requirements:

Prefer a minimum of 3-5 years geriatric case management and or geriatric social work or related experience as an RN.

Comprehensive knowledge of the long-term care system and services/resources including housing in the South Georgia area.

Strong work ethic.
Outstanding interpersonal skills.
Ability to interact effectively with staff, families, and other long-term care professionals.
Demonstrated ability to lead change and effectively manage conflict and crisis
situations.

Maintain current professional license and provide updated copies annually.
Excellent written and verbal communication skills.
Excellent planning, organizational, budgeting and interpersonal skills.
Experience and understanding of government and entitlement benefits, Social Security, Medicare, Medicaid, etc.

Valid driver’s license and proof of insurance required.
Excellent working knowledge of Microsoft Office and computer skills.
Travel throughout the South Georgia area required.
Salary determined based on experience and credentials.

Must complete a background check.

Job Types: Part-time contract 15 to 20 hours weekly

Salary: $18.00 to $25.00 /hour depending on experience.

Susan's nursing background and experiences are amazing - she's been an EMT, Paramedic, as well as having spent years in the ER, ICU, Radiology, public schools and as a company nurse at a chemical plant. She's seen it all!!

Susan is a Georgia native who was born in Willacoochee. Although she left GA, she's returned to make this her forever home. She and her husband live in Nicholls with their two dogs, Bobo (Jack Russell) and Clyde (black Lab).

Susan and her husband enjoy attending swap meets, flea markets, and their true passion - attending Volkswagen (VW) events. Her latest love is a ‘56 black VW beetle Meyers Manx! She’s a true connoisseur of VWs.

Introducing - Catherine Marino, RN, BSN

Catherine is a registered nurse with over 40 years of experience in the nursing field. She supports clients of Lambert Elder Care as an Elder Care Coordinator or ECC.

When you select the support of a Life Care Plan with Lambert Elder Care Law, your ECC will conduct a care assessment to assess your current medical needs and will coordinate with medical and health providers to provide guidance and advocacy. Your ECC will explain medical terms, help you understand medical treatment and medications, and coordinate care options to conserve your financial resources.

Catherine enjoys being a point of contact for our families and developing relationships with others. Her 40+ years in nursing attest to her dedication!

Catherine currently lives in Hahira with her two long-haired, English Cream Dachshunds. Duke and Daisy are 9 years old and are a little spoiled. The brother and sister pair love treats, belly rubs, and visits from the neighborhood children!
