Dr. Tibor Forai started his dental carrier at the age of 15 by training in Dental Lab Technician high school in Serbia. Patient came in with sensitivity to cold, sweet and biting on this tooth.
Based on the hydrodynamic theory of pain- there is pulpal fluid being forced through the dentinal tubules or a crack in dentin by a non bonded poorly fitting restoration- in this case- palatal amalgam-
The amalgam and tiny crack in dentin were removed. The crack in enamel was left because enamel has a crack stopping mechanism built into it. As says- "Cracks in enamel are biomimetic!"
The tooth was restored with a fiber reinforced composite as the dentin replacement and Z100 as the enamel replacement.

Sometimes we come across teeth with very large cavities. We can use a caries indicating solution that colors the cavity (denatured collagen) red for a more predictable clean out.
It's best to get all the decay out, but in some cases that will lead to a root canal. Root canals are sometimes necessary- especially on necrotic teeth, or teeth with inflamed pulps, but on a vital tooth with a normal response to cold testing it can and should be avoided.

This patient came in with a fractured large DO filling on a premolar.
I decided to make a semi direct restoration because the cavity was too large to restore directly (probably why the original restoration broke).
1st photo- initial situation with a rubber dam in place
2nd photo- caries removal end point after air abrasion
3rd photo- dentin sealed and resin coated and enamel etched
4th photo- the making of a semi direct inlay with teflon tape in place under the composite. The composite was bulk filled and polymerized out of the mouth and placed in a toaster oven at 120 Celsius for 10 minutes
5th photo- inlay cemented and excess removed with polishing disks
6th photo- tooth restored immediately after cementation
7th photo- palatal surface of tooth immediately after cementation
The color should equalize after the teeth re-hydrate. @ Beverly Hills, California
How to make a tooth with decalcifications and a single white line crack:
Some people have white lines on their teeth, and they need to be replicated in composite when restoring:
1. make palatal enamel shell
2. add the dentin layer with mamelons
3. add white stain onto dentin
4. add opalescent layer
5. add half of the facial enamel and white stain to replicate the crack
6. add the other half of facial enamel

This 11year old sweet girl was born with defective enamel on her front teeth. A combination of the little holes in the teeth and a lack of oral hygiene made this young girl very self conscious about her appearance.
She was teased at school and it took a toll on her confidence. Her mom wanted porcelain veneers- but I decided to go with a more conservative option and not use a dental drill on the teeth. We cleaned her teeth and bonded over the pitted defects.
She was so happy with the results she started crying, but now she knows she has to take care of them- because if she doesn’t brush and floss- all of this was for nothing. @ Los Angeles, California

Skateboarding is a dangerous sport when it comes to teeth.He face planted on the concrete leaving his teeth broken.
The patient had just gotten a new job and had to go to work the next day and was very concerned about making a good first impression.
Confidence and first impressions are very important, so I had to be very careful with restoring these teeth. I bonded two teeth and reshaped the existing crown a bit.
Bonding the front teeth is a aesthetically challenging task. It required a lot of practice if the result is going to be believable- like it is a real tooth.
The patient wanted a veneer- but since he is very young- I decided to go with bonding and a veneer some time in the future. He said: OK, lets do it!
He showed up at work the next day and no one even knew anything happened, besides his lips being a bit beat up.
Bonding can be a great success when treating fractured teeth, but you have to be careful to go to a experienced doctor.
It was a pleasure to see my patient go from sad to happy within minutes. Bonding is a good alternative to veneers, especially in young people.
This wonderful young woman was referred to me because she wanted to improve her smile.
Congenitally missing teeth and misaligned disproportional teeth created visual tensions when looking at her smile.
The midline was out of place and the teeth did not follow the lower lip,
the teeth just didn’t match the great cheerful personality she has, and
it was sad to hear that it took a toll on her confidence because she is such a great girl.
I felt that she deserves to have a great smile, and because I have done a lot of cases like hers knew I could help her.
We planned the case together and executed very precisely. I will show the planning process in another post.
Here you can see the transformation and that she now has a smile she was meant to have.
She feels confident and ready to take over the world, and I could not be happier for her.

Water polo has brought me so much in life- including the opportunity to get a free college education, and stay in the United States! Thai is a throwback photo from about 8 years ago- best times!
