Following the World Health Organisations publication for guidelines on chronic low back pain in clinical practice, we are proudly continuing to provide appropriate evidence based care in accordance with these WHO guidelines.

In 2020, 1 in 13 people (619 million people) suffered with chronic low back pain, which is a 60% increase since 1990.

A comparative study of the dose-dependent effects of low level and high intensity photobiomodulation (laser) therapy on pain and electrophysiological parameters in patients with carpal tunnel syndrome - PubMed LLLT and HILT in conjunction with exercise program are effective in reducing pain and improving median motor nerve conduction studies of the patients with CTS. It seems that high power and low fluence laser therapy is better than LLLT and exercise interventions to treat these patients.

NEW PAPER AFFIRMS LIMITED ROLE FOR SURGERY IN SPINE CARE. Spinal surgery for all forms of back pain (other than for radicular pain and disability from neural compression, cancer, infection or gross instability) is unsupported by clinical data, and the broader evidence base for spinal surgery in the management of LBP is poor and largely ineffective, says a new study published in the Medical Journal of Australia.

Instead, the paper concludes, chronic low back pain should be managed with a holistic biopsychosocial approach of generally non‐surgical measures.

This narrative review focused on studies of any design involving adult patients (aged ≥ 18 years) with axial pain affecting the region of the lumbar spine. The focus was on patients experiencing axial LBP secondary to non‐specific degenerative changes such as facet arthropathy, disc degeneration and soft tissue abnormalities.

“The evidence supporting spinal surgery for the treatment of LBP in the absence of neural compression, infection, cancer, or gross instability is sparse and contrasts with the increasing frequency at which this surgery is being performed,” the study reports. It goes on to note significant bias from sponsoring industry and device manufacturers, where low quality and poorly-designed studies reach conflicting conclusions.

The study concludes: “Surgical intervention for LBP has increased substantially among Australian patients, and disproportionately among those with private health insurance. The contribution of industry toward this increase, and their role in the conduct of published research, requires further scrutiny.”

WFC Secretary-General Richard Brown commented, “Chiropractors can play an important and cost-effective role in the non-surgical management of many spine-related disorders. Hands-on care, such as spinal manipulation or adjustment, exercise prescription, lifestyle advice and education, and other rehabilitative approaches, can all contribute positively to patient outcomes.”

“The paper’s recommendation of a holistic, biopsychosocial approach to spine care is also a powerful endorsement for chiropractic’s inclusion in tackling the global burden of spine-related pain and disability.”

It’s your Age! Or is it?

Have you ever had an ache and been told by friends and family “It’s your age, you’ve just got to put up with it”?

Or “There’s nothing we can do, it’s age-related”?

Just this week a patient told me he was just old, and asked what should he expect at his age.

Well, there are somethings that can’t be stopped or reversed, but some aches and pains are unnecessary.

So how do you know the difference?
Usually arthritic pain starts gradually, it’s bothersome at the start, but not agony. It starts when some of the cartilage covering the ends of the bones roughens and becomes thin, then the bone thickens. So the pain is not sudden.

As time goes on (we’re talking months and years here, not days) the bone at the edges of the joint thicken and form bumpy bits called spurs or osteophytes. That’s why arthritic joints look a bit fatter than normal.

Of course as all of this wear and tear happens it can cause pain, but it also causes a change in the way the joint works, which means the muscles can get tight, and the joints above and below have different strains put on them.

So, not only does the arthritic joint hurt, you’ve now also got pain from the changes in the muscles and other joints. And these can be helped with treatment. Muscle strains and joint pains can be treated with osteopathy, and we can give you easy things to do at home to help keep the area mobile. You’d be amazed at how many patients think their pain is caused by arthritis when it’s only a muscle strain.

So, don’t sit there and blame your age – get the right exercises and treatment. You don’t have to put up with it!
