Breast examination - When NOT to do

25/07/2023

When it comes to guidelines on screening.the global scenario has changed from annual mammographic screening to binneal ( ACS) or once in three years ( NICE)
With lack of cumulative data in Indian cohort on screening guidelines,we have limited studies which talk about screening

Teachers are the building blocks of society In a awareness talk with teachers of a school
We had a wonderful Time talking to them, making them feel on a manniquin and sharing their perceptions on emotional health
The onus of maintaining good health for our families and the responsibility to take care of the futurists, teachers have a great but tough role to play
Not only they need to be physically, but mentally and emotionally strong To create a better world
It was our sincere effort to interact and offer the best
Even a little change is a step towards the better world

Why it is important to upgrade your skills

Learning is an art as is teachingEven more important when you are the care givers and you want to do best for your patients
Perforator flap oncoplasty for breast reconstruction is a skill achievable by consistent efforts, thorough homework and in depth knowledge. But you need enriched soil, a true mentor and teacher for the flowers to bloom, to learn the skill so indispensable
I am highly grateful to my teacher Dr Anand Koppiker Sir 🙏 to provide me the platform to learn. . Learning cannot happen like EKLAVYA CONCEPT It has to be to be thorough, rigorous and phasic. It has to be constant The learning must continue inspite of the certification.
The skills are acquired on a constant basis. Enriched platform of learning must have all teachers with upgraded skills to spread the knowledge
With the academic platform provided by ISOS, lots of Indian students are gaining to upgrade their skills to become BREAST SURGEONS

I am highly grateful to my teacher Dr Anand Koppiker Sir 🙏 to provide me the platform to learn and grow. It's not important to be present physically to teach someone. Learning can happen with EKLAVYA CONCEPT .
He is the one to jump start thought process during my course of Mch Oncoplasty with him
He helped with all of his resources to complete my 13000 word dissertation on perforator flaps. He has always encouraged me and more importantly I feel motivated every time I think of him
The journey of 3 years of Mch Oncoplasty was not easy. He has given this opportunity to all Indian students to learn, interact and achieve skills which are not possible because of our family constraints
Happy to share that I passed my 3 year modules with ISOS ( UEA, U. K) It has been a great experience with learning of innovative methods, rigorous training with academic and guidance in practical skills
Blooming doesn't happen without proper soil

Few lines I wrote sometime back

In this time of trouble
When waves of scare and threat ripple
Making this world a cripple
Situation as bad as A Global War
Sweet memories that are charred
My friend ,,May this be a short furore
With prayers and blessings that pour
Irrespective of your country, color of skin
May you, your relatives and kin
Stay healthy and blessed
Come out in flying colours from the test

Defying the laws of nature is a crime
Earth is a holy shrine
A lesson learnt from divine
Brotherhood and mankind must prevail
All negativity We must exhale
Let's realize this in time
So that We,you and dear ones remain forever Fine

YOGA means reuniting body, mind and soul. This is a channel to take one out of the MRIGTRISHNA of external influences. Yoga is a spiritual art and we cannot derive results from our personal efforts alone, but primarily from the capacity of surrender or letting go of our individual limitations.
Deepening our awareness of our inner experience through yoga practice can develop a perspective where not much credence is given to negative habits of mind.

My take on nepotism

who s the one to be blamed
The accused? A BIG NO!
It is we, the individual , the society who sits on everything as a silent spectator, the individual , the one,who accepts to be as a scapegoat to bring home the bacon
No one in the world is as blind as a Bat
But generically, we accept to follow the trail, to hang on to those Born with a blue blood
The battle should start the day one senses the favoritism, nepotism
The society must learn to retort , reward and react the day one apprehends it
We must learn to admire the real talent, the honesty and dedication
The society must learn to retaliate silently at the outset instead of buttoning the lips till it breaks the Camel s back
With society,I refer to all the colleagues, family, friends and acquaintances
Till the day we achieve this enlightenment, to participate in whatever way, to make the nuisances bite the dust;;
Some Sushant Singh rajput somewhere will keep committing suicide, few will vanish silently and others will have to accept this as their destiny

Screening survey-2 29/04/2020

This survey is meant to assess your breast cancer risk This is based on validated models used in clinical practices to assess risk in general population
Cancer awareness in India takes a backseat because of the insufficient funds available with government, lack of awareness and motivation
In U. K, NICE guidelines propose screening starting at the age of 50 years in an average risk woman
The US TASK FORCE also recommends screening after age of 50 years
Due to the shift in the age by a decade at which maximum number of cases are diagnosed, Indian population screening starts at the age of 40
This survey has seven short questions to assess your risk
The results will be communicated to you by mail
Please write your contact ( mail I'D or phone number) in the age column

Lessons learnt from lockdown

Nothing is more important than health
In the last month,we spent hours confined to home,we explored new ventures,we jiggled at times, refused to admit that we needed to be locked inside our homes.
But, one lesson learnt

HEALTH IS WEALTH

The global majesty ,USA crashed down to mercy
The innovational patriarchs of EUROPE were helpless,
India holds back it's greener pastures because of the sensible decision- PREVENTION IS BETTER THAN CURE

What prevents us From taking care of our health
What prevents us from utilising the nanosegments of lifetime in screenings or precautionary measures
It is just our own inhibitions of mind

Do we need to be reminded again that the screening is a major stakeholder in preventing the ruins from Cancer. It might just help escaping the spin -offs and get a handle on it.
Just like you came out of the COVID CRISES
But,do we need to be shaken for every cause
So, getting the tests and screening takes nanosegments of time ,but it's worth it

You can mail any of your queries to - [email protected]
You will receive a response from our team in 48 hours

Why is covid testing important during your hospital admission for any treatment
1) covid virus is an airborne virus with incubation period of 5-6 days however it might be upto 14 days( WHO)
The virus load is highest in upper respiratory tract (nose and throat) early in
the course of the disease.,that is, within the first 3 days from onset.
If someone has contracted infection during this period, they might not be symptomatic But might be Still carrying infection.
They are potential transmitter of infection to others during this period
During this period, also known as the “pre-
symptomatic” period, some infected persons can be contagious. Therefore, transmission from a pre-symptomatic case can occur before symptom onset.

Also, the hospital might need to take appropriate measures for the safety of other patients and medical personnel

2) what if you test covid negative once, it is possible you may contract infection later at any point of time so it's important to take safety measures even if negative on test

3) Specific instructions for pregnant women- CDC says that The virus has not been detected in amniotic fluid, breastmilk, or other maternal
samples.So,Mother-to-child transmission of coronavirus during pregnancy is unlikely, but after birth a newborn is susceptible to person-to-person spread.

Pregnant people have had a higher risk of severe illness when infected with viruses from the same family as COVID-19 and other viral respiratory infections, such as influenza.( CDC)
In limited studies, COVID-19 has not been detected in breast milk; however we do not know for sure whether mothers with COVID-19 can spread the virus via breast milk.{ CDC)

I am a breast cancer specialist, oncologist and oncoplastic surgeon
Cancer and Covid- which is the lesser of the two evils.
Cancer is treatable, cure rates are high when diagnosed early
In the recent times,we all have experienced a wave of panic, scare, threat and insecurity. Even the wealthiest and the wisest were not spared of the turmoil. The situation was uncontrollable but avoidable,as all of us understand. So,we could still take control of the situation when the World was topsy turvy.
But, the victory lies in realisation. The earlier the realisation of self protection,self- security, the better. The situation matches that of any health issue or disease, say Cancer. The wisdom lies in self awareness, realisation of the importance of awareness and identifying the symptoms before the ship sinks.Self-awareness , self- examination , frequent check ups, insurance back up in deemed times of trouble, identification of related symptoms -all of these are key factors to early diagnosis and hence complete cure.
Hence,it is wise to be prepared for the difficult times to save you from a lot of emotional, functional and financial stress and biases on treatment decisions in cases of need. It’s not always necessary to have a Lockdown to learn lessons for the future.
