Covid-19 SINOVAC Vaccination

Dear Valued Patient,

We have been given instructions by MOH not to give out any more first dose SINOVAC vaccines with immediate effect due to the depletion of stocks for the 31 approved private healthcare institutions.

As such, all appointments with effect from 12 Aug 2021 for the first dose SINOVAC vaccine will be cancelled. We sincerely apologise for any inconveniences caused.

We will be opening up the link for the 2nd round of registration on 30 June 2021 (Wed) at 12pm.

Please note the following points regarding the SINOVAC Vaccination

Individuals under 18 will not be allowed to take the vaccination

The cost for the vaccine is $25/dose, total cost for 2 doses $50

Our clinic policy is to give the 2nd dose 3 weeks after the first dose of the vaccine

Please DO NOT fill in the form if you are below 18, or unable to make it for the second dose 3 weeks after.

The second form will be closed when we have reached our maximum capacity. Thank you for your patience.

To curb the community spread of COVID-19, Chua Medical Clinic and Surgery would like to introduce the following safety measures in our clinic:

- All visitors must check in via SafeEntry before entering the clinic.
- Patients seeing the doctor will also be asked to register with the MOH PRPP system to minimise the risk of infection.
- A new thermal scanner at the registration counter will be used to measure the temperature of all patients, visitors and staff for indications of fever.
- Anyone with a fever would be asked to sit at the dedicated ARI waiting area till the doctor is ready to see them.
- To further prevent the spread of this virus, please make use of the hand sanitizer located at the registration counter.

Our clinic staff will ensure compliance with the Safe Management Measures issued by the Ministry of Health following the circuit breaker period.

In this war against COVID-19, in order to support the government and community of Singapore,

Chua Medical Clinic and Surgery, being a PHPC clinic, is part of the SASH (Swab and Send Home) program.

Patients who fulfill the criteria as specified by MOH for swabbing under the COVID-19 SASH program will be able to have their swabs done at our clinic and sent home with the necessary medications and medical certificate. Singaporeans, PRs and Work permit holders (excluding domestic workers) will be able to claim this under the FSS scheme.
