Epidermal cysts (aka sebaceous cysts) are caused by obstructed pilosebaceous glands or traumatic implantation of skin cells into the dermis (inner layer of the skin). Some cysts have an opening on the surface that look like comedones (black heads).
What can you do if you have an epidermal cyst?

Antibiotics may be helpful if a cyst becomes inflamed or infected by bacteria. Antibiotics are not useful if the cyst is not inflamed.

The most definitive way to treat a cyst is to remove it completely by surgery. If a cyst is inflamed, removing it will provide faster relief from pain and swelling compared to antibiotics. Cysts removed completely are less likely to recur.

๐symmetry - if one half of a mole looks different from the other.
๐orders - if the mole has irregular or poorly defined borders.
๐olors - if the mole has varying colors such as shades of tan, brown, black, blue, red or white.
๐iameter - if the mole is larger than 6 millimeters (though some can be smaller)
๐volution - if the mole looks different from other moles or is changing in shape, size or color.
๐irm - if the mole feels firm
๐rowing - if the mole is increasing in size.

Advancing Knowledge in Fungal Skin, Hair and Nail Infections.
On 29th October 2020, Dr Koh Hong Yi gave a talk on Fungal infections of the Skin, Hair and Nails to an audience of more than 100 GPs and specialists. This is part of Dr Koh's contribution to inter-professional education, to share new developments in fungal skin infections with medical colleagues.
Fungal infections of skin, hair and nails are some of the commonest problems seen in the community. While most skin infections can be treated with over-the-counter creams, there is an emerging trend of drug-resistant fungal infections which need newer or more potent medications to be cured. Hair and nail infections, on the other hand, almost always require oral medications to resolve. Delayed treatment may lead to permanent hair loss or nail deformities, respectively.
If you have been managing a fungal infection that is not responding to anti-fungal creams, speak to your doctor if an alternative diagnosis or treatment needs to be considered.
We had a wonderful, interactive session yesterday evening live on Lazada. Together with Ms Ursula Soh (Founder of Eczema Support Group Singapore) and Jamie (from Ego, sponsor of this event), we answered questions from you on what triggers eczema especially during this pandemic, how we can protect our skin, and what are the ingredients of a good cleanser and moisturizer. Atopic Eczema โ Improving Outcomes with Proactive Treatment Approach
On 4th August, Dr Koh Hong Yi delivered a webinar on Optimizing Management Strategies in Atopic Dermatitis. The talk was attended by more than 500 doctors (including dermatologists, paediatricians and GPs) from Singapore, Malaysia, Indonesia, Philippines and Vietnam. In the session, Dr Koh covered topics ranging from the essentials of good skin care, food allergy and dust mite allergy.
The main focus of the webinar was on adopting a Pro-Active approach to atopic dermatitis (atopic eczema). Most treatments adopt a Reactive approach โ patients apply topical medicines only when eczema flares. Latest research shows that even in normal looking skin in atopic eczema, there is low-level inflammation brewing underneath the surface of the skin. If this is not treated, eczema flares easily with minor triggers. A Pro-Active approach targets this low level inflammation, delaying the time to the next flare up, giving patients more days of non-itchy skin and a better quality of life. Pro-Active treatment is safe, easy to follow, and proven to be effective in clinical trials.
The webinar ended with an engaging Q&A session. Many doctors were interested in how a proactive approach can improve the care of their patients with eczema. Dr Koh thanks the Dermatological Society of Singapore and Leo Pharma for facilitating this educational event.
On 16th June 2020, Dr Koh Hong Yi was invited by Bioacademy to give a web-based lecture on Psoriasis - Types of Biologics and the Art of Counselling. In the audience were doctors, nurses and pharmacists who are involved in treating patients with severe psoriasis.
Biologics have been available to treat psoriasis since 2002. But many patients are still unaware, or afraid of this "new" class of therapy. Most biologics are more effective, and often safer, than conventional medications. It is not uncommon now for patients on biologics to achieve clear or almost clear skin! Furthermore, psoriasis is now known to be associated with other medical disorders like coronary heart disease (heart attack), stroke, diabetes and obesity. It is even more important that we treat psoriasis and reduce the risks of having these health problems. There are now more than 7 biologics for psoriasis available in Singapore. How to choose the right one for every individual patient was the focus of the webinar.
The webinar ended with a lively question and answer session that ended beyond the scheduled 1 hour.
Singapore Gradually Exits Circuit Breaker
Singapore will exit the Circuit Breaker in phases starting from 2nd June 2020. The Ministry of Health has provided our clinic with guidance on what Dermatology specialist conditions we can see and treatments we may provide from 2nd June onwards:

2. Moderate to severe acne
3. Severe or worsening pigmentation disorders
4. Inflammatory skin conditions eg. eczema, psoriasis

Skin Problems & Skin Care Tips with Prolonged Mask Wearing
Prolonged wearing of personal protective equipment (PPE) can have detrimental effects on the skin.When the skin is exposed to high temperatures and humidity for a long time, the upper most layer called stratum corneum becomes weakened. This can lead to painful fissures or even ulcers. A study on healthcare workers found that 97% reported skin problems with PPE.
In an interview with Singapore's mainstream Chinese newspaper, ่ๅๆฉๆฅ, on 12th May 2020, our consultant Dr Koh Hong Yi lists the types of skin problems one may encounter with face masks:
1. Acne mechanica โ worsening of acne due to prolonged occlusion of hair follicles.
2. Maceration and erosions โ from humidity, pressure and rubbing from mask straps or wire strip at the nose bridge.
3. Contact dermatitis (eczema) โ either from irritation or allergic reaction to mask fabric.
4. Dermographic urticaria โ hives or wheals in people who are prone to hives from rubbing against the mask.
Some skin care tips one may try include:
1. Avoid wearing a mask for prolonged periods if possible. If you need to, stay in a cool environment to reduce sweating.
2. Apply a light moisturiser to the face, so that it doesn't clog your pores but also will not affect the seal of your mask.
3. Where the mask rubs against the skin (eg. on the nose bridge where the wire strip is, or on the cheeks against the straps), you can put a thin layer of protective dressing such as DuoDERM(r) Extra Thin on the nose or cheek.
4. Treat any underlying skin problems such as acne, eczema or urticaria (hives) so they don't flare up with mask wearing.

In a world first, researchers from Lombardy, Italy, have published them findings on how Covid-19 looks like when it affects the skin.
Examining 88 patients, they found that 18 patients (20.4%) will have a skin rash. Some of them develop rash at the onset of disease. The rash usually affects the body (trunk) and is slightly or not itchy at all.

Dr Koh Hong Yi was invited by the Psoriasis Association of Singapore to speak on Scalp Psoriasis. The scalp is one of the commonest areas to be affected by psoriasis. Scalp psoriasis can cause itching, pain, flakiness and social embarrassment to our patients. Dr Koh touched on the various types of scalp therapies and basic scalp care that all patients can apply.
Dr Koh Hong Yi gave a public talk in Mandarin on Psoriasis and latest treatments. This is a yearly event organized by the Psoriasis Association of SIngapore, under the auspices of the International Psoriasis Council.
Dr Koh Hong Yi was invited to speak to a packed GP symposium on "Bad vs Benign growths on the skin." Many thanks to Dr Constance Teo and Singhealth Head and Neck Centre for organizing and the invitation.
