Dr Goh Boon Kee delivered the Keynote Lecture on the “Advances in the Understanding and Treatment of Vitiligo” to the Japanese dermatologists at the 5th Meeting of the Japanese Society of Vitiligo at Sendai, Japan last weekend.
The participants were impressed by the knowledge sharing. Medical science has made significant advances in the understanding of vitiligo pathogenesis in the past decade, leading to the discovery of novel therapeutic targets. Critical mediators that lead to the destruction and disappearance of melanocytes include interferon gamma and Janus kinase/STAT signalling. This discovery forms the basis of using JAK inhibitors (including Opzelura cream) as a novel therapeutic intervention for vitiligo. Early studies showed that combining JAK inhibitors with narrowband UVB phototherapy provides better treatment outcome than JAK inhibitors alone. However the clinical effectiveness of these interventions still warrants further evaluation, and head-to-head comparison with existing therapeutic drugs are lacking.
When medical therapy fails in vitiligo, surgical intervention can be considered if the condition is stable. Non-cultured cellular grafting (NCCG) is a state of art intervention that has stood the test of time over the past 3 decades and surpasses tissue grafting in efficiency. A small piece of donor ultrathin split skin graft can yield enough epidermal cells to re-pigment vitiligo 5 to 10 times its size. And this procedure is safe and can be completed in an outpatient setting within 3 to 4 hours.
Dr Goh Boon Kee recently shared his expertise on the management of melasma with the Taiwanese dermatologists and aesthetic physicians at the 23rd Spring Meeting of the Laser and Photonics Medicine Society of Taiwan.
The mainstay of treatment for Melasma remains with topical lightening creams and oral tranexamic acid.
For melasma lesions that are refractory, intervention with lasers can be attempted. Skin Rejuvenation through dermal remodeling is the latest concept in lightening Melasma. This can be achieved using fractional carbon dioxide laser, pico-laser rejuvenation or even microneedling with radiofrequency.
Dr Roland Chu was very privileged to be involved in a Cibinqo event during the recent World Congress of Dermatology. Along with five other dermatologists from around the world with a special interest in atopic dermatitis, the event hosted over 95 dermatologists and health professionals from 15 different countries, during which Dr Chu shared his experience in the use of this new class of powerful drugs (JAK inhibitors) in the treatment of moderate to severe atopic eczema.
Extremely proud that the World Congress of Dermatology is being held in Singapore for the very first time! Dr Roland Chu had the honour to chair this excellent symposium on the use of dupilumab in atopic eczema. With esteemed speakers from China, South Korea, Hong Kong, Thailand, Malaysia and Singapore, it was a great evening of learning from our shared experiences on the use of dupilumab which has truly transformed our management of moderate to severe atopic eczema.
