Youth Transition Coordinator
Job Description
Job Title: YOUTH TRANSITION COORDINATOR
Reports To: Executive Director
FLSA Status: Non-Exempt
Classification: Full Time
Hours: Monday – Friday 9:00-5:00, occasional evenings and weekends
Position Summary:
This position is responsible for providing leadership and expertise in programming for youth with disabilities. The person is responsible for collaborating and establishing rapport with local/state entities, particularly with schools, as well as community partners. Services are provided to youth and their families to support independent living goals through best practices and partnerships with schools, service agencies and community organizations. This position also runs the EmpowerYOUth program, a monthly in-person meeting for young people with disabilities to connect. This involves planning agendas and possible speakers for these meetings.
Essential Duties and Responsibilities:
1. Create programs and services that meet consumers’ independent living needs and interests.
2. Work with youth individually and in groups.
3. Work with youth to develop measurable goals and review regularly per the Independent Living Plan.
4. Ensure that programs are operating efficiently, are consumer focused and within budget
5. Collaborate with community organizations to ensure inclusive, best practice opportunities are available.
6. Continuously work on outreach for youth programs.
7. Work with other CDE staff on collaboration efforts in programming. Assist other staff with their events.
8. Maintain effective working relationships will all CDE staff as well as with schools, agencies/service providers to ensure effectiveness of services.
9. Promote the Independent Living Philosophy in the community.
10. Document all contact with consumers in database according to Center guidelines.
• All Entries must be made by 5:00 every other Friday
11. Other duties as requested by Manager or Director.
Supervisory/Management Responsibilities None
Qualifications:
Experience, Education, and Licensure
Bachelor’s Degree preferred in disability related field. Minimum 3-5 years’ experience working in disability field, preferably with youth. Experience organizing programs and facilitating activities for youth with disabilities required. Experience as a trainer/educator is helpful.
Knowledge, Skills and Abilities
Must have the ability to build trust, rapport and work effectively with consumers with disabilities, their families, community organizations and agency representatives. Must be creative and energetic. Demonstrate understanding of consumer control and direction; Should be self-motivated and highly committed. Possess excellent verbal and written communications skills; Have access to transportation for travel within the service area (Franklin, Delaware, Union and Licking Counties)
All candidates must pass a FBI/BCII background check if position is offered.
Understanding of and commitment to Independent Living Philosophy is required. Personal experience with a disability given high priority.

The Center for Disability Empowerment (CDE) seeks a part time Youth Coordinator. This position
provides leadership and direction to youth with disabilities to develop independent living, self-esteem,
self-advocacy and leadership skills. The position requires self-motivation, high energy and creativity.
Experience in working with youth is required and special emphasis is placed on experience within the
