Introducing Friends to Friends!

We had the happiest morning at Metro Diner! Thanks to the Cosmopolitan Club, three of our volunteers were able to bring three of our friends for breakfast and community!

After breakfast, one of our friends wrote us an email about how much the morning meant to her- "It was wonderful to be able to get out and not be going to a medical appointment. I felt normal again. I have missed being able to get out of the house to do something other than going to an appointment. It was so nice to meet new friends."

We’re looking for a part-time Patient Care Navigator to join our team. The Patient Care Navigator is responsible for conducting home visits and performing needs assessments to determine gaps in patient care. In addition, conduct phone calls to those seeking resource information, make referrals for other service needs, and build rapport with patients and their caregivers to maintain trust in the agency.
Skills:
Exceptional communication, computer, and writing skills
Proficiency with Canva or other online graphics programs.
Knowledge of the Meta platform and use of a calendar to schedule content.
