In many cases, a brow lift is a highly effective method for the static suspension of the lateral brow. Brow descent, influenced by time and gravity, is often most noticeable in the lateral portion of the eyebrow. When this is the primary concern, I typically recommend addressing brow positioning before correcting any skin excess. I was trained to prioritize the shape of the eyebrow over its height, ensuring that the medial and lateral ends are at approximately the same level. However, some patients prefer a more dramatic “cat eye” look, which can be achieved through artistic modifications of a standard lift. The pyriform aperture implant is an easy way to achieve premaxillary augmentation. This implant, in a carefully selected patient, addresses the nasal base, upper lip, and nasolabial angle. Some of my favorite results are after mole removal. As simple as the procedure sounds, mole removal places potentially unsightly scars in the center of attractive faces and must be taken very seriously. In many cases, laser resurfacing of the wound can further enhance results of excision and closure. While mole shaving and lasers can generate acceptable short term results without the risk of longer scars, much of the lesion likely remains after these alternatives and can grow back over time. Well designed incisions and meticulous scar management nearly guarantees that the lesions are gone forever and in many cases, at the expense of inconspicuous scars. In this case, two moles were removed, including one on the right tear trough and one on the left cheek. The results are 5 months out and continue to improve.

Blepharoplasty, commonly known as an eyelid lift, is a surgical procedure aimed at enhancing the appearance of the eye area. It can involve rejuvenation or reshaping of the upper, lower, or both eyelids. Many individuals seek blepharoplasty to address concerns such as puffy eyelids, a perpetually tired appearance, or prominent shadows on their faces.
At our practice, blepharoplasty procedures are tailored to preserve the patient’s appearance while addressing their specific concerns. Dr. Zelken emphasizes the importance of eliminating shadows rather than simply excising tissue. For instance, in lower lid blepharoplasty, excess fat causing festoons may be repositioned to enhance the cheeks and efface folds.
The procedure can be performed in the practice’s procedure suite under local anesthesia. Dr. Zelken utilizes advanced techniques, such as the Lumenis AcuPulse laser, for upper eyelid lifts. Patients may undergo the procedure with oral sedation and local numbing, though twilight sedation by a board-certified anesthesiologist is also available.
Recovery typically involves sutures removal after one week, with bruising minimized through the discontinuation of aspirin and alcohol. Most patients achieve social acceptability within two weeks, aided by cover-up techniques.

See the facelift scar? Exactly. Nowadays, it is tremendously easy to manipulate images, and I pity patients who have to evaluate social media results at face value. All I can offer is my (typed) word that these results are not manipulated- and would balance the self-acclaim with the real warning that individual results may vary.

The best lip lifts are the ones you can’t see.

Buccal fat removal and reduction remains one of our favorite options for subtle facial slimming and sculpting. I have little reason to believe that its absence would be missed in one’s later years, as fat pad removal also plays an important role in my facelift practice. Take home message: a low hanging and dynamic buccal fat pad is not classically attractive. If, after removal, other features like perioral mounds become more prominent, then fat grafting or fillers to below the cheekbone (submalar) can restore that lost volume, albeit in a more desirable and aesthetic location. A quick nod to for the post inspiration. I really enjoyed a similar post of his from last week celebrating the craft of our work. Indeed, after 5 to 6 hours it can be tempting to close up quickly or delegate the closure to another team member, but this often overlooked and underappreciated step is our signature, our final touch, our encore. It is the greatest evidence of a surgeon’s skill and a good scar cannot occur without a masterful closure under minimal tension. This photo was taken today after a 5 hour deep plane facelift.

Doing what Dr. Zelken does best. Deep plane facelift. Although it’s not for everyone, the deep plane or composite facelift technique is best for midface elevation and effacement of nasolabial folds. It is our preferred choice for most younger facelift patients who do not have significant jowls, skin redundancy, or fuller necks.

Sick of smile line fillers? Following up on yesterday’s post, yes, the answer was: Implantech’s silicone peri-pyriform implant. According to the manufacturer’s website, this device is “a premaxillary implant designed to augment the perialar area around the pyriform aperture [of the nose]. Adds anterior projection to the premaxilla and provides a platform for the nasal base. In the case shown ☝️, a was desired to provide tip projection, but this was not feasible without building up the foundation of the nose first. With her mild retrusion (red dotted line), the was used to open up the angle between the base of the nose and the upper lip, efface the deep nasolabial folds, improve the facial bow (green dotted line) and provide a stable foundation for cartilage grafts used to project and rotate the tip. This device can also be used for facial aging, where the same bony changes can and do occur around the base of the nose, and comes in many flavors (ePTFE, silicone, polyethylene) and designs. These implants can also be custom manufactured or carved to size. Dr. Zelken is one of very few plastic surgeons who routinely uses this type of implant.

Does anybody know what this implant is called? Or what it’s used for? Hint number one: it is placed without invisible, incisions or scars, hint number two: it is the second most commonly used implant by Dr. Zelken. Can you spot the facelift, , corner lip lift, and blepharoplasty scars? With all the hype about facelifts, and all the new variants- vertical this and aura that, it is easy to lose focus on the big picture: the final result. I personally find it funny that prospective patients and intellectuals are more concerned about what we call our procedure than how we close the skin- or more importantly, how our patients feel afterwards. I take great pride in my closures and the inconspicuous scars I typically achieve. Also, eliminating stigmata of facelift surgery should never be ignored. In 2024, we resolve to continue to make important strides in facelift surgery for the Newport Beach community and so far beyond.

Somebody needs to see this. can be a trying procedure, particularly because it takes so long for “the dust to settle”. One of Dr. Zelken’s mentors taught him that it takes 1-3 months for the upper third of the nose to “de-swell”, 3-6 months for the middle third to do the same, and up to a year for the tip to go away for good. But by a month, patients can look and feel great, compelling many to wonder... is this the final result? We often see so many before and after photos, but sometimes it’s nice to see after and afters. In this case, the one month result versus the one year result. Sure the one month result is acceptable, but the tip is bulbous and there is supratip fullness. Only in a year are the final results manifest (refined tip, subtle dorsal hump, thin skin). Hope this helps someone in that 1-3 month awkward phase :).
