This young gentleman came to me for a hybrid rhinoplasty. He had a long history of difficulty breathing through his nose, which got in the way of his exercise and sports. We discussed different treatment options including both surgical and non-surgical and he elected to undergo a septorhinoplasty to straighten his nose and improve his breathing. At the same time, he wished to smooth out the bump on his nose and wished to keep a strong straight nasal bridge. Here he is 3wks after surgery with a much straighter nose, strong and smooth nasal bridge and improved breathing. He is still a little swollen but he and I are both very happy with the result! Another interesting case for Skin Cancer Awareness Month! This gentleman came to me for reconstruction of a large cheek defect following Mohs surgery by .adam_ for an early melanoma. There are two amazing things about this case that I wanted to highlight!

1) .adam_ performed Mohs surgery for melanoma in situ which is a cutting edge technique very few dermatologists perform for this type of cancer. This approach is very technical and minimizes the tissue removed while at the same time excising all the cancer.

2) The reconstruction I performed was developed by for cheek defects. Although the reconstruction was large, this midface flap places incisions along natural facial subunits as well as restores cheek volume to prevent hallowing and contour irregularities.

While my patients and I are very happy with these results, please take this as a reminder to wear sunscreen and protect yourself from the sun. Skin cancer is the most common cancer in the US, and although very treatable, the entire treatment process can be very scary and stressful. Wear your sunscreen!

This gentleman came to me for a scar revision following a motor vehicle accident. He sustained a complex forehead laceration that was repaired in the emergency department resulting in a significant asymmetry in his medial eyebrow position. The eyebrows are incredibly important in conveying genuine emotion and this asymmetry really took a toll on his confidence. This patient came to me for evaluation and closure of a complex lip laceration after sustaining a dog bite while at work. During the injury, a portion of his upper lip was removed and unable to re-implanted. Here he is 3 weeks after I repaired his laceration in the office. He is healing up very well and his scar will continue to fade over the next year.

I performed a rotational mucosal flap from inside the mouth to reconstruct the pink portion of her lip to maintain her lip volume, contour and shape. Here she is 1 week after surgery and already healing so well! In no time, her lips will be back to normal!

Let’s do a side-by-side comparison! Photo #1 CO2 laser resurfacing and #2 Dermabrasion. Both are excellent treatment options for wrinkles around the mouth with similar downtime. Typically, I prefer CO2 laser for more mild to moderate cases and dermabrasion for severe cases with deeper wrinkles and more sun damaged skin. Here are two of my patients 8 weeks after their procedure. I love their transformation!

This is a fun little flap! I like to call it the others call it a 🚪 flap! This flap allows me to take skin and tissue from behind the ear and pull it forward to the front of the ear through a small opening in the cartilage. In order for this tissue to survive, it has to be dissected from the surrounding tissue without compromising the blood supply to the flap. Here is a patient who came to me for ear reconstruction following for a skin cancer of the ear canal. He healed up so well just after 4weeks!

Dermabrasion—one of the many ways to treat lines around the mouth—uses a special device to gently and carefully "sand down" the top layer of the skin to reveal the underlying normal, healthy skin. This procedure can also be used to smooth and contour other skin irregularities such as scars or sun damage spots. Ear reconstruction after skin cancer removal is challenging as the ears anatomy is complex and has limited mobility. This gentleman had part of his auricular rim missing after Mohs surgery. A helical rim advancement flap was done by making cuts above and below the defect to mobilize the helical rim and bring the edges together, creating a natural contour to the ear.
