Mr. Fix It- neck lift and facelift fix🔧

Revising a prior SMAS facelift and neck lift with my INVISIBLE ACCESS EnigmaLift®️ for Carrey, who is 51 and traveled in.

We also did a full upper and lower eyelid rejuvenation🙌🏼

It’s incredibly difficult to fix and revise a lift like this, let alone doing it just with an invisible access, behind the hairline incision.

You’ll notice that the lift in that deep plane was quite powerful, even enough to fix a “pixie ear“ deformity with her ear pulling down.

6 week progress for Jessa (42) after my COMPLETELY SCARLESS EnigmaLift®️ lower eyelid rejuvenation and scarless ptosis repair!

I love the photos here because it really lets you zoom in on the fine details, which is what eyelids are really all about!

The first thing to notice is that her eyelids are just slightly more open. A bit wider and a bit brighter. Interestingly, this is something that I suggested to her that she had not necessarily noticed in the same way.

This is called ptosis. Hers was symmetrical, most are not, but you can see what it does for her overall look afterwards.

My fellow surgeons out there will recognize how challenging this is, as Jessa has very distinctly shaped eyes, and I did not want to change that! That’s always a risk with any lid surgery, and it takes a special understanding to keep things looking natural and as they were.

The most obvious change is, of course, in her lower eyelids and midface. Jessa has had lower eyelid bags almost her entire life, so seeing this for now is life-changing!

She had a strong, genetic predisposition to these lower eyelid bags, as many patients do, and as I commonly see.

This made her an incredible candidate for my COMPLETELY SCARLESS EnigmaLift®️ lower eyelid rejuvenation!

With this procedure, I was able to completely and three dimensionally reconstruct her midface fat pads, her lower eyelid bags, and use those to hide her eyesocket (orbital) rim and tear trough.

She was out and about after one week, looked even better at two weeks, and now at 6 weeks is looking incredible!

She still has some healing progress ahead, and she will only look better and better.

She took full advantage of my full optimized recovery protocol, including regenerative medicine, complete with her own stem cells! 🧬

This is a NATURAL hairline! 🤯

This is what FUE should look like - you’d never know that he had a procedure to achieve this, and you never will in the future.

These are FUE hair restoration results at 14 months for Brendan from LA, who is 45 years old.

FUE is a method of hair restoration that takes individual hair follicular units and extracts them one by one from calculated donor sites on the posterior part of the scalp and strategically places them in areas of need to increase density, restore hairlines, and improve the overall framing of the face.

My signature EnigmaLift®️ with so many subtle details and nuances! 🤩

For this beautiful woman, we focused heavily on the shape and fullness of both her brows and upper lids.

I paired that with a smoothing of her lower eyelid and cheek junction (where filler had failed), followed by tightening and repositioning of her lower eyelid margin.

Genetically, she had very full upper lids and eyebrows, which we needed to respect as we addressed her aging changes.

For me, that meant finding all of the subtle nuances that had changed, including the positioning of her eyebrow fat pads, her intraocular fat pads, and even her lacrimal gland.

I repositioned all of those structures, NOT subtracting volume and cutting them out, but getting them back to their native locations.

That is a very difficult proposition, and certainly isn’t the norm for most surgeons when addressing eyelid aging!

For her lower lids, I first started by removing prior filler manually.

I then took a similar approach to her upper lids by utilizing the volume that she already has and repositioning it, as opposed to just cutting it out, which is again not the norm.

She had very full lower eyelid fat pads, and I was able to use those fat pads to reposition into her cheek, simultaneously taking care of her eyebags and her tear trough.

Through this scarless approach, I then lifted her midface fat pads, and the combination of fat pad repositioning and a midface lift eliminated any need for volume restoration with fat transfer!

She had some inherent positional laxity in her lower eyelids that can lead to a sad look in the lower eyelids with aging, so I addressed that simultaneously, while future proofing her from any changes in her eyelid position with time.

I’ve said it before, and I’ll say it again: I don’t know that I’ll ever get over how incredible it is that I can do all of these things scarlessly!

5 years of aging backwards! Can you tell what we did?! SWIPE for some of the finer points ✍🏼

I love these types of results because you can tell that the person looks so much better, but it’s really hard to put your finger on things until those finer details are pointed out.

They look just like themselves, just a bit rejuvenated and optimized. Never transformed🙅🏻‍♂️

Lorraine (58) trusted me to rejuvenate her eyes first, which is heavily focused on re-volumizing and reshaping her upper lids and brow fat pads.

Why do bad eyelid surgeries commonly lead to a complaint of not seeing “yourself” in the mirror anymore?! 🧠

Eyelid revision surgeries are certainly one of my special skills.

I feel very fortunate and grateful that I can provide these types of impactful results🙏🏼

Eyelid revision surgeries are very nuanced, detailed, extremely challenging, and they drive my internal fire and competitive spirit to absolutely optimize the situation! 🔥

It is really common to hear that people do not recognize themselves in the mirror anymore, they do not feel like themselves.

It’s a very interesting phenomenon, and it’s related to core parts of our brain in the temporal lobe that utilize eyes for recognition, including recognition of ourselves! 🧠

Changes in eye shape, eye slope, eye size, and symmetry can throw off the self recognition centers.

Imagine what other people see, to boot!

For the patient I was working on today, Michele, she had several of these factors contributing to her issue.

This multifaceted nature makes the procedure even more challenging, even in just recognizing what those subtle changes are at her new, post-operative baseline.

Often there are issues with lower eyelid position, retraction, eye slope, and in her case an undiagnosed and missed ptosis after her prior surgery.

As I prepare for these challenging cases, I have visualized and planned through them in multiple ways: using both process visualization and outcome visualization.

This scrub time is my final opportunity to finish those nuanced visualizations, and I get the added benefit of tactile, corpus callosum 🧠 stimulating feedback while I do it!

Wendy had a complex and dynamic eyelid ptosis, upper lid aging with lacrimal gland ptosis and hollowness.

Non-surgical lifting and tightening devices - what are they actually doing?

There are so many of these now (which often tells you that none of them work well)

Most of them rely on heat to dehydrate layers and contract them, which is flawed logic in the long-term. Now we’re seeing new devices targeted at the muscles in the face and neck.

All of that excess heat applied deep to the face causes issues with our normal facial structure.

Changes can occur in the fat pads, in the junction of the skin with the fat, and with the skin itself.

These layers get glued together, they become fibrotic and scarred, and the blood vessel health of these areas is significantly compromised and insufficient.

As many surgeons know, when we are doing true deep neck contouring (as opposed to just superficial SMAS work), enlargement (called hypertrophy) of the deep neck musculature can be a significant contributing factor to undesirable contours and a blunted neck angle.

Putting a device on these already enlarged muscles to further “contract“ them is going to lead to a whole new adventure for us as we are actually addressing aging necks.

It’s a bummer that these devices just don’t really work in the first place, but the irony gets thick when these devices affect what ultimately happens when patients take the definitive approach to achieve the results they were after initially.

A pile of sand on your s**g carpet?! 😳

Walk through this analogy with me as we explore the roles of filler dissolution and manual filler removal.

It’s really popular that you’re seeing a lot of celebrities getting their filler dissolved right now, but is that the only option? Is that the best option? How do things work to get filler out when you don’t want it there anymore?

Getting rid of filler is one of the most common questions that I get from patients talking about hyaluronic acid fillers, which is mostly what we’re going to talk about today.

We can “dissolve” these HA fillers using an enzyme called hyaluronidase that you inject around the filler, helping to break it down.

When we’re talking about getting rid of filler and the best ways to do it, we also have to talk about the ability to manually remove the filler. When it comes to managing filler, I’m usually doing it around surgery and I’m using surgery to help manage that filler as part of the process.I do this different than most of my colleagues - how to manage filler around surgery is totally based on expert opinion and this is my expert opinion on the best way to do it. I don’t heavily dissolve that filler before surgery, I don’t want my patient coming in altered from having a bunch of enzyme utilize to dissolve the area that we’re working.

The reason I don’t like to dissolve first is because I can manually remove a lot of that filler when it’s necessary. For some people, manual removal is a key part of their filler management plan.

The analogy I like to use here for my patients who have filler is that you have s**g carpet and on that s**g carpet, you have a mound of sand sitting there.

That’s quite literal, because when I’m manually removing these hyaluronic acids, it actually looks kind of like sugary or sandy as I’m taking it out on that s**g carpet.

So that analogy of the same on the carpet just helps to illustrate that it’s often best to utilize multiple tools to get rid of that problem!

🇧🇷 Complicated and significant revision EnigmaLift®️(an essentially scarless revision!) for a colleague from Brazil!

She is 67 years old, and I’m so honored that she made the trip from Brazil to see me.

She’s a renowned physician in Brazil and knows who’s who throughout the world.

All of this invisible work was very intricate, revising prior procedures: a prior brow lift, upper and lower eyelid surgery, facelift, and neck lift. *plus significant prior filler*

You can see the beautiful changes in her upper eyelid, with more open eyes from ptosis repair, optimized upper eyelid volume with my stem cell-rich fat transfer, a revision of her prior upper lid surgery, and correction of her lower lid surgery.

This EnigmaLift®️ revision complex has left her eyes looking fantastic, younger, softer, more relaxed, and more symmetric.

Her face and neck revision were very challenging. I was able to utilize ZERO incisions around the front of her ears to revise her prior facelift and neck lift with my Invisible Access EnigmaLift®️

🚀 This is what she was seeking and why she came to me specifically.

We could have done a more robust revision for more significant results, but we were definitely able to take advantage of the 80/20 rule, Pareto principal, to achieve incredible results with very, very minimal input.

Her prior neck lift left “lumps” in her neck - if you look closely, you will see this ALL THE TIME!

For her neck revision, we lifted, sculpted, and contoured her deep neck contents, including her submandibular glands, deep fat, and deep musculature.

That did exactly what we wanted to restore the contour, profile, and surface quality of her neck.

My techniques change the thought paradigm for this answer.

Some surgeons perform too large of procedures on too young of patients, others refuse to perform any procedures on patients who are clearly ready for some degree of lifting, solely based off of their numeric age.

They are treating a number instead of treating the patient.

For younger patients, I take a very different stance and approach to this than most other surgeons - an invisible access approach, as I love to utilize the Pareto Principle, putting in 20% of the work to get 80% of the results 🙌🏼

This allows me to get amazing results out of a bigger procedure, with basically ZERO scars and no evidence that I was there whatsoever.

I just saw a patient who had a deep plane facelift at 32 years old. 😬

She has scars around her ears, and she’s unhappy with the results from her prior procedure.

In my opinion, it was the wrong procedure for her to have in the first place. Now we’re talking about revising that facelift because she doesn’t like it.

For me, that’s where I’m going to apply my invisible access EnigmaLift®️, a minimally invasive procedure with NO visible incisions afterwards, and I’m able to access the brow, the mid face, and the jawline to get the appropriate amount of lift for somebody who’s earlier on in the aging process.

This can be the perfect procedural mix of beautiful results with minimal invasiveness for someone in their 30’s, 40’s or sometimes 50’s!

Everybody loves Bill! How could you not?!

Bill is my typical male patient. He is 67 and a BAMF - he’s an awesome guy, a super athlete to this day, and he wanted to address his eyes.

I want to achieve his goals and do it essentially SCARLESSLY, unoperated, naturally.

Together, we chose an incredibly minimally invasive set of procedures to refresh his eyes and make him look more awake and improve his symmetry.

We are going to even the contour of his upper lids, then through the same access, we will address his sinking brow fat pad, no extra incisions! For his lower lids, we’re going to scarlessly approach his eye bags and reposition the fat pads to fill his tear trough.

Heart rate variability may be the most powerful metric to predict your performance readiness🧠

HRV tracks the recovery of your central nervous system and serves as a window to your neurocognitive capacity on that day, along with your readiness for physical performance and output.

HRV is a beautiful tool to optimize your daily life, whether that is exercise and recovery, looking at the impact of nutrition, or gauging your performance for a big event coming up.

It’s incredible that your heart can even serve as a window into your central nervous system to know how neurologically recovered you are and what’s happening with the various states of your nervous system.

In our nervous system, we have our parasympathetic “resting and digesting” part of our nervous system and we have the sympathetic, which is its counterpart, also known as our
“fight or flight” nervous system.

Our heart rate gets controlled (or left to randomly beat on its own) depending on which nervous system state is
predominant:Parasympathetic = high HRV.

When we are in resting and digesting, we are totally recovered, things are good, and our heart rate is not really being controlled by our nervous system.

The variability between our heartbeats is quite significant when left to beat at its own pace; things are not
metronomic. Sympathetic = low HRV.

When we get into fight or flight, the beats get much more regular, more metronomic, as our nervous system takes over control of our heart rate.

So we can look at the variability between your heartbeats and know how recovered you are, with that window into what is happening inside of your central nervous system.

Wearables like your your band, your watch, and even your iPhone will tell you your HRV - and these devices rely heavily on HRV when they are telling you your recovery and readiness.

I use HRV very strategically and purposefully when getting ready for all of my surgeries🙌🏼
