Strengthening and conditioning of muscles and tendons is imperative to ensure good endurance, rate of force development, force absorption and most importantly injury prevention. (More time running less time on the sidelines). If those reasons aren’t good enough for you, let’s see if this will work…. It will make you run faster and longer.

RETURNING TO SPORT AFTER KNEE SURGERY

As an athlete the last thing you should worry about is- where you place your foot; does my knee feel stable in this position? and can my leg support the momentum of my upper body? I mean let’s take soccer for example, You have to keep your eyes on the ball, locate teammates be aware of charging opponents and make a decision on whether to run through the gap or pass the ball to your teammate. The last thing that should be on your mind is “can my knee cope if I put weight through it?” These should be Automatic and done subconsciously. To do this it is imperative that your Physiotherapist take you through the stages of Motor Learning to reach the Autonomous Phase (other phases: Cognitive and Associative) we know this phase as muscle memory, state of flow, second nature, habitual, unconscious. Reaching this stage can’t be rushed and all criteria must be met.

Here Reece is going through one of the tasks in the Autonomous Phase. The “Colour Drill”. Different task have to be completed, and the last thing he can worry about is how stable his knee is.
Phase1: Toe touching colour cone on verbal command.
Phase 2: Toe touching colour cone on verbal command, while continuous catching and throwing ball
Phase 3: Toe touching colour cone on verbal command. Identifying colour displayed on iPad, while continuous catching and throwing ball.

* Please be advised that these exercises should be guided and decided on by your physiotherapist, after determining that your phase of rehabilitation and comfort level allows for this.

Single leg hops:

You frequently hear people say that once an athlete sustains an ACL tear, they will never be the same again (regards to performance as good as they were before the ACL injury). Why is that? Is this really the truth or are we as physiotherapist failing to return these athletes back to elite performance due to inadequate rehabilitation.

How many ACL repair athletes were cleared to return to sport without rehabilitation tasks that forced them to take off, land or hop on a single leg?

Studies showed that a large percentage of athletes did not return to the same level of performance due to Kinesiophobia. In simple terms they were scared to land or hop on the operated leg. Studies also should that 2 years out the athlete will still try to land on the non-surgical leg.
Single leg training is so important as loading improves absorption during walking and running gait mechanics, but more importantly the ability to use the leg without fear.

Single leg hop tests are also imperative to clear athletes to return to play.
These tests include:
- Single hop distance
- Single leg hop distance
- Lateral hop test
- Diagonal hop test
- 6 meter hop test

Here are a few simple single leg hopping tasks.

Please be advised that these exercises should be guided and decided on by your physiotherapist, after determining that your phase of rehabilitation and comfort level allows for this.

Today we celebrate World Health Day and we will focus on how much exercise is recommended per week for adults.
As per The American Heart Association and CDC; for overall cardiovascular health, at least 150mins of moderate-intensity aerobic exercise per week or 30mins for 5 days OR 75mins of vigorous aerobic exercise per week or 25 mins for 3 days, or a combination of the two. This needs to be carried out with moderate to high intensity strength training for at least 2 days per week.
For reducing blood pressure and cholesterol, an average of 40mins of moderate to vigorous aerobic exercise 3-4 days per week is required.
We often hear that there is not enough time to exercise, but remember that 5 mins a day is better than nothing.
Happy exercising!

Knee force absorption

The force that is put through the knee is 1 and a half times your body weight during walking and up to 6.9 times your body weight during running. This makes the ability to absorb forces through the knee very important. The cartilage that absorbs forces between the knee joint is called the meniscus, and we have minimal influence on the structure’s capability on force absorption.
Things we can influence are the muscular structures supporting the knee and helping in force absorption. The two most known and superior muscle during force absorption are the quadriceps and hamstrings improving the strength and absorption qualities of the muscles will give better support to the knee joint.

Please swipe to see a few of absorption style exercises for the knee.

Please consult your physio, to make sure that you are ready and in the phase of rehabilitation to do these exercises.

Eccentric phase of the hamstring

The hamstring muscle lengthens as it slows the leg down before the foot strikes the ground.
Most hamstring injuries during running and acceleration occur during the lengthening phase of the hamstring muscle. So why when we rehabilitate the hamstring after a strain do, we only focus on the shortening phase of the hamstring?
We should be strengthening the hamstring during the eccentric phase and adapt it to better withstand these forces during lengthening and the transition back to the concentric phase (shortening).
Eccentrics exercises also prevents tissue disruption during lengthen forces during high-speed running.
The most common eccentric exercise known are the Nordic curls, however this requires you to put big loads through the hamstring what if we not ready for the loads required during Nordics.

Here are some eccentric loading tasks that could be used during the rehabilitation of hamstring strains:

*Please be advised before trying these exercises that you see your physiotherapist to be assessed, to make sure that you are ready and can tolerate these loads.
