Participation Support Services

We strive to help adults with physical disabilities to live as independently as possible.

Participation Support Services Brantford receives its funding assistance through the Hamilton Niagara Haldimand Brant Local Health Integration Network (HNHB LHIN) via the Ministry of Health and Long Term Care. The funding from the LHIN provides approximately 80% of the total annualized budget for Participation House Brantford. The LHIN does not provide funding for any special equipment or capital

Last week, our PSS staff and residents volunteered at Gifts Of The Heart Brantford, lead by our very own PSS Board Member, Paul MacDougall, and Supervisor, Andrew Fyfe. This amazing volunteer kitchen prepares meals for local schools and our PSS team was more than happy to participate.

A big thank you to Paige Johnson and Carter’s Training Centre for playing Santa and hosting a stocking stuffer drive for Participation Support Services. Paige enlisted the help of her family, friends, riders, as well as the St. George community to fill the stockings of all our residents with gift cards, personal items and fun activities. Enough donations were collected to fill the back of an SUV. We cannot thank you enough for helping to make this Christmas special for the people we serve. Also in the picture is the lovely Secret who joined the festivities to pose as our reindeer.

On behalf of PSS and the people we serve, thank you to these dedicated volunteers and everyone else involved to make this event a great success!

The BBNOHT is proud to announce the release of the BBN OHT Health Navigator app! This app was designed to help patients and caregivers in Brantford, Brant and Norfolk explore local and provincial health services and more seamlessly access their personal health information in one convenient location!

The app is free to use and available in both Apple and Google Play stores by searching “BBN OHT Health Navigator.”

The BBN OHT Health Navigator allows users to search for local and provincial health services by categories:

Immediate crisis support
Primary Care
Hospitals/Urgent Care
Mental Health and Addictions
Housing and Outreach
Home and Community Resources
Older Adult Services
Caregiver Resources
Public Health
Provincial Resources.

Users can further narrow their search by service type and link directly to provider websites for more information. Additionally, by clicking “Free Health Advice” in the app, users are taken to the provincial resource Health811 (formerly Telehealth) where they can receive free health advice or information from a qualified profession 24/7!

Users can also access their personal health information on the app through tools like ConnectMyHealth, a new digital health solution that provides patients with an online, single access point to view their health records from participating hospitals in southwestern Ontario, including Brant Community Healthcare System and Norfolk General Hospital Simcoe Ontario

In order to ensure we are providing a valuable experience for the user, the Brantford Brant Norfolk OHT welcomes suggestions and feedback. Tell us what you like or how we can improve the app by clicking “Learn More” in the app or emailing [email protected].
