Eric Arzubi figured psychiatrists were more familiar with the consequences of su***de than people in other professions. He was wrong.
A firefighter told him today that he's been called to more su***de-related DOAs than he cares to remember. DOA stands for "dead on arrival."
Psychiatrists get to care for people who have survived su***de attempts. Firefighters answer the call after someone has completed su***de.
He spoke with Ryan Moore, Captain of Engine 7 in Billings, Montana. Ryan has been the first responder to su***de-related DOAs of people of all ages.
Ryan has a daughter, so he was especially troubled by the surge in su***des among teenage girls during the COVID-19 pandemic. He noted that many of them had lost all hope after big family blow-outs or tough romantic breakups.
When Ryan first arrives on the scene of such a tragedy, he keeps an emotional distance. But when the victim's family members start to arrive, things get real and he feels the full impact of what happened.
Distraught parents and siblings beg him to resuscitate their loved ones, but he's already given it his best shot, and it's just too late. Once he's made the official declaration, the reality hits families, and another wave of emotion washes over him.
Eric called Ryan because he read that firefighters were more likely to die from su***de than in the line of duty from 2014 to 2020. He had overlooked the extent of the trauma witnessed by firefighters.
Until now, Eric only thought about police or clinical teams in ERs when considering burnout and rising su***de rates among first responders.
The Billings Fire Department launched a peer support specialists (PSS) program in 2017, and three months ago, a chaplain joined the Department. These resources were put in place to better support firefighters in the wake of critical incidents.
Stigma remains a barrier to receiving that support. Ryan admitted that he and his brothers still don't really appreciate if or when they need the help.
Ryan's insights remind him that it's not always obvious who in our communities need support the most.

Accelerating Access to Quality Care
Iโm tired of excuses from some of my colleagues in healthcare as to why change canโt happen more quickly. While we sit on our hands, weighing the pros and cons of innovation, our patients continue to suffer.
We are in a mental health crisis because our systems have failed us. Period. I founded Frontier Psychiatry as a way to continue pushing for improved access to care for all our citizens, no matter where they live.
