One of the best parts of my job is the ability to regularly work with the Mount Sinai Facial Plastic Surgery Fellow (who has completed residency, and is voluntarily pursuing additional training in Plastics for a year.) is a remarkably talented, calm, poised and conscientious surgeon. Every case we’ve done together has been value add for both of us. I am extremely proud of him for being offered/accepting an academic position at Emory in Atlanta. They really don’t come better - humble, thoughtful, meticulous, technically gifted, and with a very keen eye. Overall such a great dude (only knock on him is as an Eagle’s fan). I’ve greatly enjoyed working with the fellow each year. I share a small part of the credit with my plastics colleagues and fellowship director . Everyone benefits from JRo’s initiatives in making this one of the more coveted fellowships Nationally.

Generally speaking, I suggest this profile appearance to female rhinoplasty patients of mine. A gradual, soft transition to the tip from the bridge. Not a slope, not a straight line, but a gently curved transition from top to tip.

Rhinoplasty for a class act of a Middle Eastern young man/ university professor. Common features of Middle Eastern noses include prominent bridge, droopy and underprojected tip related to weak cartilage, and wide nostrils. A large number of structural cartilage grafts are generally needed to stabilize and shape the nose. Here, augmentation was also done above the hump to help lengthen the nose.

I aspire to offer world class rhinoplasty surgery for patients. This was a difficult revision for a sweet female patient with a poor outcome from prior surgery in a foreign country. Her own rib cartilage was used to lengthen an overly rotated nose, and add forward projection to a flat tip. Her bony bridge was narrowed, while stabilizing the middle portion.

I almost never post early post op photos, but this lovely patient wanted me to share her profile at 2 weeks. We both think she looks great! I frequently use a strong stabilizing cartilage graft at the base (caudal septal extension graft) in female rhinoplasty, which allows for long-lasting control and positioning of the tip.

Delighted for this pretty young lady, 6 months following rhinoplasty motivated by breathing issues. Her attention to fine detail parallels mine, which called for nearly perfect ex*****on. Given her very thin skin envelope, various camouflage techniques were used (temporalis fascia, finely diced cartilage) to give the nose a soft and smooth finish. The changes are relatively subtle individually, but make a real difference when taken as a whole.

Was really special skiing out West with two of my earliest childhood friends (see photo three as proof that we literally potty trained together). We’ve mostly grown out of the awkward high school phase (photo 4 on a high school Israel trip - numerous f***y packs involved, awkward hair cuts, oversized athleisure, ignore the ci**rs). These friendships are irreplaceable. Other than immediate family, noone understands my past, my idiosyncrasies, or my value system better.

Cleft rhinoplasty for a young man born with cleft lip/palate deformity. Such patients have associated nasal deformities that skew the septum and tip, and distort the nostril on the cleft side (see base view).

Asian rhinoplasty (female). The general theme is augmentation of the tip and bridge, typically using rib cartilage, while substantially reducing nostril width. The bridge here was augmented using finely diced cartilage in a dissolvable biological glue graft (DCGG).

This kind man developed rhinophyma of his nose in his middle-age years, which was a source of undue attention and insecurity. Rhinophyma is a condition in which the glands of the nasal skin thicken and are chronically inflamed. He is 6 weeks following combination treatment with electrodessication (shaving with a heat wand) and laser resurfacing. While still healing, he reports being much more comfortable in his own skin over the holidays.
