Face lifting as the main part of solving the rejuvenation puzzle ,more often than not ,will not complete the task .
Many colleagues show very early before and after results , in young beautiful woman with minimal aging touting superior results with some fancy named , so called new , innovative procedure ( vertical restore , deep plane , ponytail or enigma lift etc ) .
Facelifting without specialized techniques to address the eyes and mouth are often incomplete . The eyes and mouth are the keys . Ever wonder why the yellow emoji reveals just the eyes and mouth? Because intense expressions of emotion with happiness or sadness /grief can be elicited with only the eyes and mouth . This is a two year result in a beautiful woman approaching 80 years of age , where a corner of mouth lift and upper lip lift was performed together with a facelift and autologous fat injections throughout the orbits and face to correct the skeletonized face (deflation) so often seen with aging .

As a medical legal consultant this is just another obvious medical - legal blunder from what should have been a routine revision cosmetic surgical procedure. Instead this case turned into an obvious malpractice case and a real reconstructive challenge .

The previous surgeon inadvertently left a retained sponge In the right breast during an implant removal and replacement procedure ( note the large * right chest mass and the foreign body removed ) . The retained sponge was neglected for over one year. She presented to our clinic without a right breast implant and a chest wall mass with severe deformity .
The opposite left breast was encapsulated .
This was a difficult reconstructive challenge as she desired a return of breast size and shape . This was no longer a cosmetic case , but reconstructive . We attempted to salvage the soft tissue
by placing a biological acellular dermal matrix graft under the thinned out skin from an indirect incisional approach and attempt a one stage silicone breast reconstruction while simultaneously removing the deep retained retropectoral foreign textile sponge , along with breast lift and left capsulectomy. The longstanding retained sponge and scar tissue was stuck against the rib cage and required meticulous laborious dissection to remove . She received hyperbaric oxygen treatments for one week in the post operative period . Note the three month result . This case represented a difficult reconstructive challenge. Surgical algorithms and cookie cutter surgical techniques become irrelevent when the surgeon must improvise confronted with a problem for which there is little to no precedent . This disaster could have been avoided with a proper sponge counting ,meticulous technique , and attention to detail.

7 DAY result after
Reconstructive Secondary Septo -Rhinoplasty while working in Dubai . This beautiful middle eastern young lady had previous rhinoplasty ten years prior and presented with the imbalance of the tip and dorsum . These photos were taken immediately after sutures were removed . She returned to work on the third day post op . No packing or fracturing of nasal bones was necessary . Notice the minimal swelling in the dorsum and the early overprojected and rotated nasal tip which will decrease over time and become even more balanced . The tip must be over elevated and slightly over rotated early on to look great long term . The entire tip was distorted with deformed cartilages and was restored with costal cartilage and multiple 20 + permanent sutures were removed . Attention to detail and respect for normal anatomy is paramount in performing such finesse secondary rhinoplasty after unfavorable previous surgery .
Revision rhinoplasty surgery is very difficult for a number of reasons : * Altered anatomy 
The first surgery has already changed the nose’s anatomy, making it harder to separate tissue layers and access structures. 
*Scarring from the first surgery can make dissection more difficult and affect surgical techniques.* Cartilage 
Revision surgeries often require cartilage to replace damaged or missing cartilage from the first surgery. 
*Unpredictable factors 
Each case has its own set of factors, making standardized approaches less effective. 
*Lack of information 
Surgeons often have little idea of what was done in the first surgery until they expose the area. It is my personal belief a plastic surgeon needs to have performed at least 200 revision rhinoplasty surgeries in his or her past , to properly analyze the problems and optimize results for these often highly complex reconstructive procedures .
