A few weeks past we posted that the flu vaccine shots had arrived, but we didn't yet have the nasal spray formulation. Those are now available, just in time for the flu vaccine season to get underway.

REMINDERS on deciding between shot vs mist:

-Flu Mist is not approved under age 2 years, but even those slightly older who might start crying once they hear they're getting a flu vaccine or those who can't sniff up the vaccine on command should get the shot. If the vaccine runs out of the nose because it's not sniffed up right after it's sprayed in, or if the child is crying so much they have a runny nose, the vaccine can "wash out" before it has a chance to take.
-You should not get a Flu Mist vaccine if you have a history of asthma attacks triggered by colds/upper respiratory infections.
-Flu Mist is reserved for our pediatric patients. If you are a parent whose insurance will allow you to get your flu vaccine from a pediatric office, we ask that you get the shot.
-Flu shots are for 6 months and older. As with last year, this year's flu shot is not egg culture-based and safe for those with egg allergies.

Keep in mind that if a child under 9 years old is getting a flu vaccine (either formulation) for the first time in their lives, they will need two doses (again the two doses could both be the shot, both be the nasal spray, or be one and then the other) separated by at least 4 weeks this season to be fully protected.

Call the office to start scheduling your flu vaccines. If a parent would like to get the flu vaccine with their child(ren) please specify that at the time of scheduling so that we can make sure your insurance will allow it and also that we can make sure supplies don't run out.

Note: we do our inventory check on Tuesday afternoons, so flu vaccine appointments are not available on Tuesday afternoons after 3 pm.

Wondering if your child is ready to stay home alone? According to the American Academy of Pediatrics (AAP), it's crucial to ensure they are prepared both mentally and physically. Here are some essential tips:

1๏ธโƒฃ Age and Maturity Matter: The AAP suggests considering your child's age, maturity, and comfort level before leaving them alone. No specific age fits allโ€”assess their readiness individually.

2๏ธโƒฃ Emergency Preparedness: Make sure your child knows how to dial 911, and has a list of trusted contacts. The AAP emphasizes the importance of clear communication in emergencies.

3๏ธโƒฃ Set Clear Boundaries: Establish rules about cooking, answering the door, and screen time. Consistency is key to following safety protocols.

4๏ธโƒฃ Practice Makes Perfect: The AAP recommends practicing different scenarios with your child, such as what to do if a stranger knocks or if there's a fire. Rehearsing these situations can build their confidence and readiness.

5๏ธโƒฃ Start with Short Stays: Gradually increase the time your child spends alone. The AAP advises starting with short intervals to help them get used to the responsibility.

Our patients have 2 options to consider: nasal spray (unfortunately not available for children under 2 years old) & a shot-administered vaccine.

Shots this season are not egg-culture-based, so those with egg allergies need not worry!

๐—™๐—”๐—–๐—ง๐—ฆ:
-Flu shot is for anyone age 6 months and older.
-Flu shot is a dead virus, so you cannot get a case of flu from the flu shot. Flu Mist is a weakened form of the virus, so you can get a slight cold, but the virus has been genetically modified to not be able to live at body temperature.
-Those with a history of asthma attacks triggered by a cold should not get the Flu Mist, but should stick with the flu shot instead.
-Though the Flu Mist is approved for 2 year olds and above, if your child is going to freak out at the idea of a flu vaccine because it's a vaccine, he or she may not be a good candidate for the Mist because crying can wash out the vaccine before it has a chance to "take.".
-Ideally, you want to receive this vaccine before the end of October

Our flu shots have arrived, but we have not gotten the flu vaccine nasal sprays yet. Stay tuned for social media/newsletter announcements about when that happens. You can go ahead and call the office to schedule flu shot appointments for September and October. If parents want to also get the flu shot, please mention that at the time of scheduling your child's appointment so we can confirm your insurance and put you onto the schedule also. Please keep in mind, no flu shot appointments on Tuesdays after 3 pm.

Impetigo is a common, itchy, and sometimes painful skin infection caused by bacteria, usually Staphylococcus aureus or Group A Streptococcus (Staph and Strep). It often occurs when the skin is broken, but it can also infect intact skin, especially in warmer months when children are outside.

๐—ฆ๐—ถ๐—ด๐—ป๐˜€ ๐—ฎ๐—ป๐—ฑ ๐—ฆ๐˜†๐—บ๐—ฝ๐˜๐—ผ๐—บ๐˜€:
- Sores and blisters around the nose, mouth, scalp, hairline, arms, and legs
- Different types include:
- - Non-bullous Impetigo: Most common; itchy sores burst quickly, forming honey-colored crusts that heal without scarring.
- - Bullous Impetigo: Rare; affects mainly children under 2 years; large fluid-filled blisters that heal without scarring.
- - Ecthyma: Untreated impetigo can develop into this deeper skin infection, leading to thick crusts and potential scarring.

๐—ง๐—ฟ๐—ฒ๐—ฎ๐˜๐—บ๐—ฒ๐—ป๐˜ ๐—ฃ๐—น๐—ฎ๐—ป:
1. Keep the area clean.
2. Apply antibiotic ointment (Mupirocin) with a Q-tip.
3. Monitor for signs of spreading or worsening infection such as:
- Increased redness
- Purulent discharge
- Warmth
- Red streaking
If multiple areas are involved or the infection is spreading rapidly, oral antibiotics may be necessary.

Have you ever heard of Fifth Disease? Its name might sound made-up, and when you learn it's caused by a parvovirus, it seems even strangerโ€”especially for dog owners who know "Parvo" as a serious pet illness. But for humans, Fifth Disease is typically mild and underwhelming.

๐——๐—ถ๐˜€๐˜๐—ถ๐—ป๐—ฐ๐˜๐—ถ๐˜ƒ๐—ฒ ๐—ฅ๐—ฎ๐˜€๐—ต
- Starts with extremely red cheeks, often mistaken for sunburn or wind-chapped skin.
- Spreads to the upper arms and legs in a lacy, red pattern.
- Eventually covers the torso and fades away in 1-2 weeks.
- Rash isnโ€™t usually itchy or bothersome.

๐—–๐—ผ๐—ป๐˜๐—ฎ๐—ด๐—ถ๐—ผ๐—ป
- Child is no longer contagious by the time the rash appears.
- Contagious in the days before the rash, with possible mild cold-like symptoms or none at all.
- At-risk groups for complications: individuals with bone marrow diseases or unborn babies.
- Notify at-risk contacts to seek medical advice.

- Rash can become more pronounced with increased body temperature from activities like playing, bathing, or sleeping.
- This doesnโ€™t indicate worsening.
- Rash won't respond to Benadryl or hydrocortisone; itโ€™s just the bodyโ€™s reaction to the virus.
- If slightly itchy, regular lotion or cool compresses can help.

Neonatal Nasolacrimal Duct Stenosis AKA "Newborn Blocked Tear Duct":
๐—”๐—ป ๐—ข๐˜ƒ๐—ฒ๐—ฟ๐˜ƒ๐—ถ๐—ฒ๐˜„ ๐˜„๐—ถ๐˜๐—ต ๐—–๐—ฒ๐˜ƒ๐—ฒ๐˜† ๐—ฃ๐—ฒ๐—ฑ๐—ถ๐—ฎ๐˜๐—ฟ๐—ถ๐—ฐ๐˜€

Parents, have you noticed yellow/green goo in your newborn's eye(s)? While itโ€™s natural to worry about an eye infection, there's a common and usually harmless cause โ€“blocked tear ducts.

Tears, made by the lacrimal glands, keep the eyes healthy and lubricated with a special combination of water and proteins. They flow across the eye and drain into tiny tear ducts, which sometimes get blocked in newborns. This leads to tears collecting in the eye and leaving behind a yellow-green mucousy drainage.

Hereโ€™s how to tell if itโ€™s a blocked tear duct:
-Whites of the eyes remain clear (not red)
-No redness or swelling between the eye and the nose

Most babies outgrow this by 7-9 months as their tear ducts widen. Meanwhile, gentle cleaning with warm, wet cotton balls can help keep the eyes clear without causing irritation. Some experts suggest gentle massage near the nose might help open the blockage, and itโ€™s worth a try!

Poison Ivy, Poison Oak, and Poison Sumac
๐—”๐—ป ๐—ข๐˜ƒ๐—ฒ๐—ฟ๐˜ƒ๐—ถ๐—ฒ๐˜„ ๐˜„๐—ถ๐˜๐—ต ๐—–๐—ฒ๐˜ƒ๐—ฒ๐˜† ๐—ฃ๐—ฒ๐—ฑ๐—ถ๐—ฎ๐˜๐—ฟ๐—ถ๐—ฐ๐˜€

These three plants make an irritating oil called urushiol that can cause a very itchy allergic skin rash as a result of contact with even just a tiny amount. Hereโ€™s what you need to know to keep your little ones safe.:

Poison Ivy: "Leaves of three, let them be." These leaves can change colors with the seasons and might have white waxy berries. It grows as a shrub or vine.
Poison Oak: Similar to poison ivy but with rounded tips and fuzzy undersides. Also grows as shrubs or vines.
Poison Sumac: Found in wet, wooded areas with 7-13 leaflets per leaf. It can grow up to 20 feet tall with drooping yellow, green, or cream-colored berries.

Symptoms: Itchy, red, swollen rash with blisters, developing within hours to a few days after contact. You may react to future exposures to these plants even if you didnโ€™t react in the past.

Treatment:
Rinse your skin and wash your clothes immediately
Use anti-itch creams like calamine lotion and steroid creams
For weeping blisters, try topical astringents (Burows, Domeboro)
Oral antihistamines
Cold compresses
Duration: Symptoms usually last 1-2 weeks.

โ†’View fireworks from a safe distance
Professional fireworks shows are going to be more spectacular, and safer, than backyard fireworks. Enjoy them at a safe distance, at least 500 feet away from the fireworks launch site.

โ†’Wave a flag (or glow stick) instead of a sparkler
Sparklers may seem relatively harmless, as fireworks go.

โ†’Protect your childโ€™s hearing
Fireworks and firecrackers can be as loud as 150 decibelsโ€”a lot louder than whatโ€™s considered a safe listening level (75โ€“80 decibels). At close distance, even one loud burst is enough to cause some permanent hearing loss, according to the American Speech-Language-Hearing Association.

โ†’Never allow young children to play with or ignite fireworks
Sparklers burn at an extremely high heat: 2,000 degrees Fahrenheitโ€”hot enough to melt some metals. Sparks can ignite clothing on fire and cause eye injuries. Touching a lit sparkler to skin can result in third degree burns.

Understanding Food Reactions: Hypersensitivity (Post 3 of 3)

The previous two types of food allergies discussed are relatively immediate in regards to onset of symptoms after exposure to the triggering food. However, food allergies can also present with symptoms that gradually develop with chronic exposure.

Delayed-type food hypersensitivity causes symptoms more consistent with chronic inflammation - chronic abdominal pain, chronic diarrhea, poor growth, bloody stools, chronic skin rashes.
-Onset is from hours to days to weeks to months after exposure due to the different type of antibodies that are triggered in these allergic reactions
-Cow's milk protein allergy seen in infants, and celiac disease are examples
-Only treatment is avoidance of the specific food triggers, no desensitization therapy is available
-Regular allergy testing does NOT check for these antibodies, blood testing for the specific type of antibody to the suspected food trigger is necessary

Understanding Food Reactions: Oral Allergy Syndrome (Post 2 of 3)

Another set of symptoms frequently concerning to parents for possible allergies is a syndrome called Oral Allergy Syndrome.

Oral allergy syndrome (OAS) symptoms are limited to rash or irritation around mouth in response to eating specific raw fruits/veggies that are similar chemically to pollens that the patient may be sensitive to.
-Onset within minutes
-Benadryl also helps treat symptoms of OAS, but symptoms also typically self resolve even without therapy within an hour; OAS symptoms can actually be avoided by cooking or at least significantly heating up the fruit/vegetable so for example raw apples might cause rash/mouth itching but applesauce or apple pie are tolerated without problems
-Even though symptoms are localized to around the mouth, they are unlikely to cause actual breathing difficulty or be life threatening
-Testing would have to be for the pollen that the individual is allergic to, and so would be less reliable before age 3-4 years old
-Typically becomes milder with time

Understanding Food Reactions: Allergy vs. Intolerance (Post 1 of 3)

There are many misconceptions surrounding food allergies.

Many symptoms thought to be an "allergy" are actually intolerance (eg. lactose intolerance, diaper rash after tomato products, diarrhea with too much fruit) resulting from a chemical reaction of substances in the food to the part of the body having the reaction.

Classic food allergies (e.g. nuts, eggs) result in hives or symptoms of anaphylaxis to various degrees:
-Usually within minutes of eating the food trigger
-If just hives diphenhydramine (Benadryl) can help symptoms, and no ER visit is necessary, but if any symptoms of anaphylaxis, epinephrine autoinjector is also needed and an immediate trip to the ER is the best course of action
-Can escalate with subsequent exposures (just hives the first time, hives and vomiting the next time, hives, vomiting, and difficulty breathing the next time)
-Can do allergy testing in infants if suspicious symptoms; note: testing won't come up positive if haven't ever been exposed to the trigger being tested for
-Some allergies have densitization therapy available

Why Reduce Triglycerides? An Overview with Cevey Pediatrics

Here are some effective ways to lower your triglycerides:
1. Aim for a healthy weight โ€“ excess calories are stored as fat.
2. Reduce added sugars โ€“ found in sweets and soft drinks, they convert to triglycerides.
3. Opt for a lower carbohydrate diet.
4. Eat a high fiber diet โ€“ includes fruits, veggies, whole grains, nuts, seeds, and legumes.
5. Exercise regularly.
6. Cut down on trans fats โ€“ avoid fried and baked goods.
7. Increase Omega-3 fatty acids โ€“ found in fatty fish.
8. Choose unsaturated fats โ€“ nuts, seeds, and avocados.
9. Add soy protein โ€“ tofu, edamame, tempeh, soy milk.
10. Enjoy tree nuts in moderation.
11. Consider natural supplements like fish oil, vitamin D, fenugreek, and curcumin.

Neonatal Jaundice is a common condition characterized by yellow discoloration of a newborn infantโ€™s skin and the white part of their eyes. This occurs due to an excess of bilirubin, a yellow pigment of red blood cells, in the babyโ€™s blood.

Physiological Jaundice: Newborns produce more bilirubin than adults because of increased production and faster breakdown of red blood cells in the first few days of life. The liver filters bilirubin from the blood into the intestinal tract. However, the immature livers of newborns often cannot remove bilirubin quickly enough, resulting in jaundice. This type of jaundice typically appears on the second or third day of life and is harmless.

Pathological Jaundice: This type appears within the first 24 hours of life and can be caused by factors such as internal bleeding, infections, malformations in the bile ducts, or incompatibility of the motherโ€™s and babyโ€™s blood. Sometimes physiologic jaundice can become pathologic if levels go up too high or too fast.

Treatments for Newborn Jaundice:
Sunlight: Sunlight emits a blue-green light that converts bilirubin into water-soluble isomers that can be excreted. The more body surface area that gets exposed to sunlight the better.
Increased Feedings: More frequent feedings can help by increasing stools, which helps to eliminate bilirubin from the intestines more efficiently.
Phototherapy: Light therapy from a concentrated light source of the perfect wavelength that is prescribed if physiologic jaundice has become pathologic and you are not getting enough improvement from sunlight exposure and increased feedings.

Stinging Caterpillars: An Overview with Cevey Pediatrics

As the weather warms up, so do encounters with stinging caterpillars! It's essential to be aware of these fuzzy but potentially harmful critters, especially if you have curious kids around. Here's what you need to know:

Stinging caterpillars are most active from March through December, so stay vigilant during these months. These stinging caterpillars, when touched, can cause painful rashes on contact!

Typically, stinging caterpillars look fuzzy, which in a childโ€™s eyes is interpreted as looking soft and fun to pet. With their bright patterns and fun-looking wave-like motion when crawling along the ground, they are extremely attractive to kids to try to pick up.

Examples include:
Puss moth caterpillars, buck moth caterpillars, spiny oak slug caterpillars, hickory tussock moth caterpillars, saddleback caterpillars, and Io moth caterpillars.

Symptoms:
May include immediate pain or pain that starts several minutes after being stung, along with a line or multiple lines of reddish-colored spots where spines enter the skin.

Treatment:
Washing the involved skin immediately with soap and water, and drying without contacting the skin (e.g., using a hairdryer). Cool compresses can reduce pain, and applying rubbing alcohol or ammonia topically can enhance the cooling sensation. If eyes are affected, irrigate immediately with lots of water.

โ€ข Eczema flare-ups classically have been treated with topical steroids (of which there are over-the-counter or prescription options) but excessive use of topical steroids can cause side effects as well as lead to dependence and withdrawal.

Croup is a common respiratory condition in young children, usually caused by a virus. It leads to swelling and narrowing of the upper airway, resulting in a barking cough and difficulty breathing.

Croup is a viral illness, often caused by the same viruses behind a regular cold.
-It not only inflames nasal passages but also vocal cords, potentially leading to airway blockage in young children.
-Peak swelling typically occurs on days 3-5, after which the cough transitions into a regular mucousy one.

Oral Steroids: Prescribed for 2-4 days, these can reduce swelling and ease the transition to a regular cough.
Cool Mist: Sit your child near a cool mist humidifier, in a steamy bathroom, or outdoors on a cool, damp night to alleviate symptoms.
Emergency Care: If cool mist doesn't help within 10-20 minutes, a breathing treatment at the ER may be necessary.

ยทFebrile seizures
- typically occur between the ages of 6 months and 5 years
- typically happen within 24 hrs of an illness
- are frightening to watch, but do not cause brain damage or affect intelligence

ยทRisk factors for recurrence of febrile seizures include:
- younger age at first seizure
- short duration of fever before the onset of the first febrile seizure
- lower temperature at onset
- and family history of febrile seizures.

ยทApproximately 1/3 of patients will have a recurrence of a febrile seizure.

REMEMBER: Reduce future fevers as soon as possible with ibuprofen/ Tylenol.

Another method for safe viewing of the partially eclipsed Sun is indirectly via pinhole projection. For example, with your back to the Sun, cross the outstretched, slightly open fingers of one hand over the outstretched, slightly open fingers of the other, creating a waffle pattern. In your handsโ€™ shadow on the ground, the spaces between your fingers will show the Sun as crescents.

Molluscum Contagiosum Virus: An Overview with Cevey Pediatrics

๐—ช๐—›๐—”๐—ง ๐—œ๐—ฆ ๐—œ๐—ง?
Molluscum contagiosum is a common skin infection caused by a virus. It mainly presents as round nonitchy nonpainful bumps varying in size from tiny pinheads to eraser-sized bumps. If the bumps are scratched open, the virus can spread. While Molluscum naturally goes away, it usually takes at least a year, sometimes 2-3 years to do so.

๐—ง๐—ฅ๐—˜๐—”๐—ง๐— ๐—˜๐—ก๐—ง
โ€ข Applying anything topically to the bumps that causes mild to moderate local irritation can trigger the immune system to respond to the irritation and in doing so also kill the virus. Keep in mind that these treatments may still take several months to result in clearance of the virus.

-this could include undiluted apple cider vinegar, or over-the-counter or prescription leave-on treatments for acne or aging (benzoyl peroxide, salicylic acid, other alpha- and beta-hydroxy acids, Differin, Retin-A, etc)

โ€ข Cantharidin is also now available to pediatricians, but does have to be approved by the insurance company. If approved, we apply the treatment here in office. Be aware that it does cause a small blister to form at the site(s) of application which should resolve within 3-5 days of forming. Follow-up treatments should be done every 3 weeks, but usually after the second to third cycle of treatments, complete resolution is expected.

Altitude sickness is a combination of symptoms that can result when you take in less oxygen than your body is used to because of travel to a high elevation (8000 ft or higher).

Acute mountain sickness (AMS) is the mildest, most common presentation of altitude sickness. If someone who already has AMS continues to ascend without giving his body a chance to adjust, more serious complications may develop.

The best way to prevent altitude sickness is to take your time when traveling to higher elevations. Therefore a road trip from sea level to Denver (5000 ft) is unlikely to result in altitude sickness, but if flying, you may want to spend a night or two at an intermediate elevation. Acetazolamide often is prescribed for adults to prevent altitude sickness in adults but for pediatric patients it technically is considered off-label to prescribe acetazolamide to prevent or treat altitude sickness.

- retreat to a lower altitude so that she can acclimate more gradually. Symptoms should subside within a couple of days
- encourage fluids to maintain good hydration status
- acetaminophen or ibuprofen (depending on age) as needed for headache
- encourage your child to take deeper and slightly faster than normal breaths

Go to the nearest ER for difficulty breathing, turning blue, or anything more than minor discomfort!
