Remember this lovely patient?  Check out my previous post for the backstory...

She has Poland Syndrome (a congenital birth defect involving one side of the chest and upper extremity). It’s typically sporadic (not inherited), and can manifest with hand/arm abnormalities on the same side as the underdeveloped or missing pectoralis muscles {near complete absence of pec major for her}.  It can also have variable underdevelopment of the ribs and subcutaneous fat on the affected chest wall {likely present in this patient also}.

📝 Many great discussion points with this case:

1. For her, the implant pocket is predetermined by her anatomy.  Ideally, this should be noted BEFORE surgery so as to discuss the aesthetic implications for her result.  For surgeons who “only do it the same way every time” (I know a few), this could be a set up for patient disappointment🥺

2.  There are inherent limitations in symmetry and cosmetic appearance of the breasts after surgery because it is not just a muscle asymmetry problem. 🏗️

3.  The surgeon should discuss fat transfer to the upper chest to attempt to smooth out the transition point where there is neither muscle nor implant.  However, in this case, the patient and Dr. Albright 🤓 elected to forgo fat and are very happy with the outcome.😉

4.  Volume and projection asymmetry will likely persist even if different implant profiles and volumes are used (for her: moderate profile 295cc on the left, and full profile 345cc on the right).  The surgeon is still limited on the HEIGHT and WIDTH of the selected round implant.  Although a larger round implant would be taller (have more vertical height to better match the left), the larger right implant would also be wider, and can start to look much wider than the left.  It’s a balance! ⚖️

📝 Clinical Vignette: 30 year old female in a very physically demanding career has noticed “asymmetry” for as long as she can remember. She is right hand dominant and, interestingly, she also feels that her right arm is stronger in the gym🏋️. She is able to perform strict pull-ups and push-ups with ease. She does not have any hand abnormalities. She would like to go from a 34B to full C, and have better symmetry. What are your thoughts and what would be your surgical plan? Why did Dr. Albright 🤓 pick this case?

This case highlights the importance of preoperative assessment, particularly physical examination and the need for your plastic surgeon to be facile with multiple augmentation techniques.
Dr. Albright was able to diagnose something that had been missed by her pediatrician, her primary care physician and her gynecologist. You can see hints of it visually, but can confirm it with physical exam (and ultrasound if needed but not necessary in this case). This anatomic difference between sides MUST be identified BEFORE surgery, as there is a good chance the surgeon would NEED to change his/her surgical plan and MUST discuss the EXPECTED aesthetic outcome with the patient. This is why it is so important for surgeons to be able to perform multiple augmentation techniques (subglandular/subfascial/dual plane/retropectoral) and adjust the plan based on the patient’s goals/anatomy.

While a thorough examination and measurements are needed for an exact answer (which Dr. Albright will perform during a consult), here are a few quick ways to see if you might be a candidate for a breast lift:

1️⃣ Use a straight edge (like a pencil or ruler) and check if your breast overhangs the breast fold (where the breast meets the chest wall at the bottom).
2️⃣ Next, check if the ni**le overhangs the breast fold with the same straight edge.

👉 If either (breast and/or ni**le) are overhanging, you might be a good candidate for a breast lift!

🏫 Starting the year off Right with some Teaching! ✔️
📣Revision implant surgery is challenging! Even the consultation is challenging as the surgeon must try to convey to the patient what factors must be surgically addressed during their revision surgery. And then convey to the patient what result may possibly be achieved (or not achieved).🫣

This is particularly true when the patient is considering not replacing their implants, as this can limit what volume, and shapes may be achievable.🤔

However, for the well informed patient, these decisions can be made with more confidence, and the surgical results are more likely to satisfy. 😉

For example, Dr. Albright 🤓 anticipated the many baseline asymmetries in this patient (seen here after her implants were removed), and attempted to account for these differences to achieve a MORE symmetric result! 🎊

This lovely 34 year old mother of 2 had bariatric surgery in 2016 and lost about 100 pounds! Although she had gained a little bit back, she had achieved a stable weight, and was still not satisfied with her physique.

She desired a lifted look with mid shape and was relatively satisfied with her current volume. She considered implants but would wait until after the lift 🏗️. For her abdomen, she wanted a more youthful appearance.🤔

Luckily, she found Dr. Albright 🤓 who recommended and performed a lift (making the ar**la smaller), and an abdominoplasty with liposuction and muscle repair. Oh, and he tailored her a fantastic belly button! 😉

📣 On this day of Thanksgiving 🦃, let's be INSPIRED! 🌟
Meet Michael Cervantes🏋️. He attends our gym () and works for Child Protective Services in Bexar County👮‍♂️. Specifically, he works with older kids/young adults in "Conservatorship" with the state of Texas (kids who are aging out of foster care). And his work keeps him busy! For example, he'd drive across state lines to check on one of his wards or pick up a runaway (Texas is a BIG state to drive across). The guy is passionate about his work!

Anyway, one day, I asked Michael if he believed any of his kids were "lost causes" (in medicine, sadly, there is such a concept as futile care)? Without hesitation, he immediately said "NO! None of them are lost causes"! 🥲

The ensuing discussion we had inspired ME 🤓! I am grateful for unsung heroes like Michael and thankful he has been able to make a profound impact on our society!

💕So I asked Michael what is one of the best ways to financially support his Conservatorship kids? He recommended donating to Bexar County Family Services.

🤔 BCFS is a nonprofit organization in San Antonio, which works with government and other community organizations to provide services and resources to families with young children, older kids after foster care or juvenile detention, and s*x trafficking victims. These services can include shelter and financial support (like low interest loans).

This Thanksgiving we have donated to BCFS in honor of Michael Cervantes' passion as he recovers from knee surgery 🔪. If you feel similarly inspired, I encourage you to show your appreciation and donate... something to someone! 🎆

✨Baby Feeding x5 YEARS...💕
This lovely 27 year old mother of 3 spent nearly 5 years nursing her babies🤱. When she was nursing, her cup size would increase to a DD. After finishing, they would shrink to an A. 🫤

📝 She desired to be somewhere in between: C+/D- with a mid to mid augmented shape. Also she noticed that her ni**les were quite stretched out and wanted to know if there was a way to make them more normative in size and shape.🤔

🤓 Dr. Albright recommended dual plane surgery with full profile 335cc implants and a ni**le reduction! Yup, it IS a thing! (Unfortunately a rather challenging thing to show here, but can be accessed on our website 🫣).
