RESCUE
My beautiful patient was getting fillers pumped into her precious little lips and her injector confused her lips for a ski slope ⛷🤷♂️
She has and now after a session of hyaluronidase followed by my procedure
The scar is extremely minimal, she has a shortened philtrum / upper lip, and she has better lip volume, shape, contour, and appearance.

My beautiful patient came to me because she had by another provider. She had puffy lower eyelids and hemosiderin staining from the injections. Laser helped the staining but the puffiness stayed.
But she ended up going through 5 session of hyaluronidase!!
I see a lot of revision patients and I do my best to take a thoughtful and careful approach. Some cases I take slowly, especially cases like this.
I used only 1 syringe between both undereyes here and she was so happy with the results she hasn't needed any more for over 1 year!

Reductive blepharoplasty is not the answer because the fat is simply removed and this will result in a more volumed-depleted state, create more hollows and a appearance, and worsen the appearance.
I also don’t subscribe to reductive blepharoplasty + fat grafting.
So, what is one to do?
with fat pad preservation, isolation, and mobility for repositioning are important. But even with these done properly, repositioning of the fat on a flat maxilla or negative vector patient may not give an adequate result because there is no adequate scaffold, foundation, and bony support.
is therefore an important adjunct in the treatment of these patients. This can be in the form of before or after , or during or after blepharoplasty as a 2nd stage based on patient desire.

A tasteful transformation for my beautiful patient who is just a short time out from surgery.
I zoomed into this super high-quality image so you can see the incision placement.
This incision went across the entire nasal base close to the natural alar-facial and alar-labial crease.
I don't place the incision in the actual crease and never in the sill.
She had a very long and so I performed a dissection and minimized tension on the closure.
She now has a shortened philtrum, a more volumized appearing upper lip that doesn't need fillers, and a beautiful 👄

Have you ever wondered why people look overfilled or unnatural even when under the care of excellent an injector?
A common issue I see is overfilled temples. One reason this happens is due to "hollow-appearing" temples in relation to prominent cheek bones (either from cheek filler or the patient's anatomy). So the patient and injector keep filling the temples to make them match the level of the high cheeks, not realizing the temple just looks hollow when it really isn't, and this cycle leads to an odd and unnatural appearance.

A patient should have what I call a double silhouette. This is natural and beautiful. More volume doesn't equate to more beauty. Having contour changes and shifts, even if they are rather abrupt, adds to the beauty of an individual.
On first glance, it looks like this patient could benefit from more filler. But if you look at her high cheekbones, you will come to realize that injecting more filler will just make her look unnatural.
Relationships are extremely important to recognize and respect. Just because you can change them doesn't mean you should!

This handsome male patient underwent a and scarless to achieve a nice arch to his brows without feminizing him and reposition the lower eyelid fat pads. This patient was hollow to begin with so repositioning all of his fat was crucial. You can also see the improvement in his cheek.

I'm so excited to share her ! I performed with through a hidden transconjunctival approach from inside the lower eyelid.
She has a very unique and atypical case of ted which has been challenging for her; between the eyelid edema, proptosis aka , aching pain, double vision, etc.
