5 different applications of spinal endoscopy (UBE) in one day at our outpatient surgery center: far lateral hnp, facet cyst excision with bilateral partial facetecotmy, interlaminar L5-S1 mcd, adjacent level to a prior fusion with a massive L2-3 HNP, and a revision laminectomy with neurolysis. All done as an outpatient efficiently in a cost effective manner.

I am often told that MIS Spine surgery and a TLIF cannot generate lordosis. I would challenge that thinking especially with a bilateral tubular complete facetectomy and subtotal diskectomy coupled with a near complete annular release. nothing fancy just 2 20mm tubes and a c-arm. I call this procedure a Posterior ALIF. I have few indications to go anterior despite having a phenomenal access surgeon. This patient was discharged 2 hrs after surgery as an outpatient.

I have been told dogmatically that full endo shines in far lateral disc herniations and that biportal endoscopy struggles. honestly i have not found this to be the case. Here is a patient with a large far lateral L4-5 HNP with severe L4 exiting nerve root compression. At the end of the diskectomy the L4 nerve root and kambin’s triangle is seen. This is not an argument of UBE versus full endo but rather a conversation about maximizing value, improving patient care and access all while reducing costs.

This patient is a world famous physician with a large cervical C7-T1 HNP with severe neuroforaminal stenosis causing weakness in the arm. I treated him with a biportal outpatient posterior cervical foraminotomy and diskectomy done in our surgery center. He was discharged less than 2 hrs after the procedure. The importance of this case is the ability to provide state of the art care to patients at a minimal expense. The total supply costs were less than $200 and I used a zero degree shoulder scope. I continue to challenge endoscopic companies to reduce their price points if they want their technology to become truly mainstream in the surgery center environment

A step by step method to treat a higher grade spondylolisthesis with an outpatient all minimally invasive posterior approach using two 20mm tubes. A limited sacral dome osteotomy, bilateral complete facetectomies and a sub total discectomy allows the placement of two 14mm 12 degree implants. Finally compression and reduction of the spondy is done maximizing lordosis through simultaneous bilateral rod reduction.

What a privilege and an honor it is to go back to your medical school as a visiting professor. Words cannot express how humbled I am to go back to the Rothman Institutue and Jefferson Medical College this week. I can’t believe it’s been 25 years since I graduated. Excited to see my mentor and close friend Alex Vaccaro as well as the rest of the Rothman spine surgeons who are all tremendous leaders.
Nothing more satisfying than taking out a massive disc herniation endoscopically! Even more gratifying and humbling is when your former spine fellow now a spine surgeon sends you his future brother in law. No greater compliment than when a prior trainee trusts you with their family member. My motto to everyone in the OR assisting me is that the person on the table is someone’s mother, father, brother or sister.
This patient was referred to me from southern Illinois. A heavy laborer that underwent a fusion over 18 months ago and had difficulty with activities. The patient never had a postoperative CT scan to rule out a non-union and was told titanium always heals. Unfortunately the patient did have a non-union. Luckily for the patient we were able to treat his non union and to correct his alignment through a single stage mini anterior fusion as an outpatient.
Another very timely article about the tremendous power and application of off label cervical disc arthroplasty. Indications for Cervical disc replacements have continued to expand and I only hope that the payors realize the tremendous savings and benefits that a disc replacement has for our patients with cervical disorders.

One of the most special compliments given to a surgeon is when a former trainee and well regarded spine surgeon refers you a patient. Even more special is when that patient is a world famous Orthopedic Surgeon and the Chair of a renowned University. Fortunately, I was able to provide an outpatient ultra minimally invasive endoscopic diskectomy. Instantaneous relief! Dr. Singh has an international reputation in the area of minimally invasive spinal surgery. He frequently teaches minimally invasive techniques to neurosurgeons and spinal surgeons from around the world and most recently was an invited guest lecturer to Saudi Arabia. He is also extensively involved in designing new minimally invasive spinal instrumentation for cutting-edge spinal surgery.

Dr. Singh is known for his excellent outcomes and his use of cutting-edge procedures. He is an award-winning researcher and has published his findings in major medical journals (more than 200 publications, presentations and book chapters). He most recently won awards for his research including Best Paper at the North American Spine Society (NASS) and Society of Minimally Invasive Spinal Surgery (SMISS) International Meetings in 2013. He frequently trains surgeons who observe his newly advanced surgical techniques in the operating room. He is also the principal researcher in several FDA clinical trials involving cervical disc replacements and motion-preserving spinal technology.
Dr. Singh is listed in Becker's Orthopedic Spine and Pain Management as one of the nation's Top 100 spine surgeons and specialists. He was also awarded the "Top 10" Doctor Award, Most Compassionate Doctor, and Patient's Choice Award from Vitals.com.
