2 year follow up for and everything looks great.

Initially she had discolored composite fillings on the in-between surfaces and a lot of natural translucency & surface stains made the teeth read slightly gray. Since she’s always on camera or competing on stage, she wanted her smile to pop while also maintaining the shapes and proportions that made her feel like herself.

Restoration of the worn dentition is in fact prevention, because it anticipates the degradation of function, limits biological damage, slows down functional damage.

The challenging part of this case was the severely darkened front tooth (previously treated with root canal surgery called apicoectomy). Metal crowns or posts (which we shouldn’t be using anymore) can create this dark band around the gum line added by the transparency of the tissue which allows show through of the darkened tooth. Sometimes there exists pigmentation of the stained gum tissue itself which can add to this aesthetic challenge. Masking with an opaque core is an option here, as long as the patient is made aware the dark root show through may still be visible. We focused on reducing dominance of the front teeth, raising the position of the posterior teeth (crown lengthening) and building out the sides for a broader, more wide/full smile with

Dental erosion is a “two speed” process. At first, the tooth wear progresses slowly because the enamel is more resistant; later, when the underlying Dentin becomes exposed, the hard tissue loss speeds up, especially if there are other associated causes (horizontal chewing pattern, hyposalivation, abrasive surfaces such as ceramic). It’s important that we as clinicians diagnose the cause of the acid erosion and have a discussion with our patients (since many are unaware that they have a condition known as gastric reflux).

In this case I was able to correct the frictional chewing pattern and add strength/ structural support to the teeth using thin and super-natural veneers.

The restorations need to feel natural so they have their best chance for a long life of service.

The aesthetics must be in harmony with the facial muscles for phonetics as well as the jaw joint.

Our back teeth serve as vertical stops and it’s important for them to come together correctly so you aren’t hitting on “slopes” which causes your jaw to slide. If you hit on the front teeth then your lower jaw slides back or it pushes the upper teeth out. We call this a “constricted chewing pattern”.

Think of the upper and lower jaws as two gears which communicate with one another. If one gear is off then you can have problems with your bite, pain in your jaw, teeth breaking etc. So when we get a chance to fix both in the proper position and pathway it makes for longer lasting restorations.

Balancing pink and white aesthetics, implants, crowns, and veneers. This patient came to me after being in temporaries for several months. As in all cases, we designed the smile according to the facial features and then placed new temporaries which provide critical information related to the patients functional movements.

It takes meticulous attention to detail and alteration of the temporaries to arrive at a final design that is not only aesthetic, but functional.

Not all are created equal and not all cases require 20 teeth. Our patient was given a few options, with the risks & benefits associated with each. We opted for a conservative option and only a few and she was THRILLED with the result.

Clean, minimalistic, ultra thin. Not changing the way she looks, simply enhancing. Not everybody needs 20 veneers.

We had a discussion about lip filler and her future goals related to maintaining lip volume or reducing. In my opinion, if you plan on dissolving lip filler, it should be done before getting veneers.

This lovely patient was dissatisfied with her veneers (the “before” photo above - that were placed by her local dentist). Aesthetically, they read dull, rectangular and lifeless. We focused on harmonizing the architecture/shape of the gum tissue as well as balancing the horizontal plane of the smile so it doesn’t appear “tilted” in the face. (Capturing the natural head posture during photography and design phase is critical here). We enhanced the shapes and alignment so they reflect light favorably, as well as aligned the midline. The shade is bright and vital, but natural. The result is a more youthful, rejuvenated

This lovely patient is one of the sweetest people I’ve had the pleasure to treat. She first presented to my office as an emergency because she fractured her veneer. She asked me if it was normal for veneers to break since this wasn’t the first time. In short, the answer is no. But first I needed to investigate why this happened to her and evaluate the way her teeth fit together. Only after this comprehensive assessment was I able to confidently execute a revision treatment that is not only beautiful, but long-lasting.

The goal was to create dominance in the centrals, decrease the width of the laterals and give her an overall more vibrant shade. We’re so happy for her!

Invisalign —> Veneers. Smile rejuvenation using 24 and replacement of posterior . The goal was to create that 10 years younger, more vital, “healthy look”. We focused on correcting the maxillary cant (horizontal plane of the smile) and “jumping” the left posterior crossbite. The teeth were built longer on her left side to balance the asymmetric lip dynamic and the overall smile within the face. As always, this design, especially the proportions and length of the teeth, was tweaked and adjusted in the temporaries.

(For any curious dentists- The diagnostic functional profile was acceptable function. She has efficient use of masticatory muscles and the envelope of function was within normal limits. I restored this case in MIP without raising the vertical dimension).

Oh yeah- this is my mother. I love her very much and I’m happy that she’s happy ☺️

My patient suffered a traumatic accident and needed several root canals across his upper front teeth. He was relieved when I told him that not all root canal treated teeth need crowns. The guidelines for how we as dentists restore root canal treated teeth has evolved!

This patient was dissatisfied with the staining and chipping from old composite bonding. She decided to upgrade to . She was seeking a subtle result so we blended four veneers with her natural dentition. Notice how these veneers mimic the natural details. One benefit to this material is that it does not stain or discolor over time. Completed in two weeks.

A smile that enhances rather than distracts. The main aesthetic challenge was correcting the tooth size proportions and aligning the midlines. Even more important than the upper and lower midline aligning is the upper midline parallel to the facial midline. In the before photo, the midline was slanted to the patients right side.
