Dr. Leary uses both VenClose and laser treatment options in his office, depending on the patient’s particular anatomy. Both VenClose and laser ablation are quick and effective for treating venous insufficiency. Both use heat to close the problem vein from the inside. Dr. Leary’s laser machine is a 1470nm laser, which is a better and more advanced option than some of the earlier lasers that actually cauterized the problem vein. The 1470nm laser has proven to be highly effective yet very gentle, and results in less post-operative discomfort, and is Dr. Leary’s choice when it comes to very short problem veins. Because it can pass through more tortured and curved veins a little more easily than VenClose, Dr. Leary usually recommends also laser ablation to patients with that condition. Patients with large straight veins in contrast, respond very well to VenClose. Dr. Leary will decide which procedure is best for you after a vein examination.

Sclerotherapy has made a major evolution with the introduction of Microfoam. Traditional sclerotherapy causes damage to the lining of the vein, causing it to collapse and making the walls of the vein then stick together. The vein is then slowly reabsorbed and disappears. Foam Sclerotherapy works in the same way only it is much more efficient. Foam is made by adding Co2 to a liquid sclerosant such as the detergent Polidocanol (Asclera). The liquid and the C02 are agitated to produce a foam mixture. The foam is then injected directly into the vein. Foam is not diluted like the conventional sclerosant, so much less is needed and it remains in the vein for a longer period of time. Foam also works better because it pushes the blood out of the vein and makes a better contact with the vein walls. This enables us to treat much larger veins with liquid Sclerotherapy.

Another advantage to using foam is that it is very echogenic, making it very easy to see on ultrasound. We can track the entire vein that has been treated. We also track the travel of the foam in a particular vein and stop the flow by applying pressure with the ultrasound probe, then flow can then be redirected if necessary. The selective treatment of veins with foam is more complete than with surgical removal or microphlebectomy.

The Varithena procedure involves injecting the material into the vein using ultrasound to help guide and monitor where the foam goes. The overall effect works to increase blood clotting and scar tissue formation and collapse the affected varicose vein, which is then naturally eliminated by the body. Blood flow is rerouted to healthier veins. Once the vein collapses, the foam stops working. Is EVLA a Safe Procedure?
EVLA is a safe and effective treatment that has a 98 percent success rate. Most patients don’t need to have a second treatment. EVLA produces no visible scarring but can cause bruising that can last for up to six weeks. Bruising and tenderness are the most common side effects after treatment, but some less common side effects include infection, puncturing and thermal damage. These risks are rare and most patients undergo the procedure with no complications.

What Is Endovenous Laser Ablation?
Laser vein removal for varicose veins is often performed with endovenous laser ablation (EVLA). During an EVLA procedure, the skin is numbed with local anesthesia and a small laser fiber is inserted into the vein. The fiber then emits laser energy into the vein, causing its walls to shrink and preventing blood from continuing to flow through the vessel. Eventually, the vein walls collapse and seal shut, and the treated vessel is naturally absorbed by the body. After the EVLA procedure, patients will be able to return home and to normal activities right away. Most patients return to work the very next day, although exercise and strenuous activity should be avoided for a few days. There is no stitching or suturing needed after this procedure.
