History of Phlebology Part II.

Unlike many of our European and Australian colleagues who have embraced and accredited Phlebology for over 60 years, Phlebology has never achieved a status of a fully recognized specialty in the US.

Opportunities and training for phlebologists have evolved quite differently in different parts of the world. It seems ironic that the US medical establishment seemingly ignores a pathological process that affects millions of Americans and will increase as our population ages. Reasons for this include increasingly centralized and restrictive healthcare funding.

American medical academics have largely ignored venous disease, and as a result, teaching institutions have failed to provide the resources necessary to develop standardized training programs and certifications. As a consequence, treatment of venous disease has fallen onto both physician and non-physician practitioners from every conceivable background, often with haphazard and inadequate training!

The American College of Phlebology (ACP) was formed in 1986 and meets yearly to advance proper treatment and training for venous disease. Proper treatment involves extensive knowledge of leg vein hemodynamics and ultrasound, which Dr. Enns is highly skilled and knowledgeable in, given his board certification in Radiology. He is an active member of the ACP and prides is providing first-rate treatment of venous disorders.

Take caution when choosing a leg vein specialist and ensure they are well trained and experienced, for both cosmetic and medical indications!

Compression therapy is such a vital part of a phlebology practice. Patients with venous disease often feel noticeably better with compression stockings, mainly because of improved venous return and minimizing venous hypertension.
Elite athletes also use compression stockings. Improvements in athletic performance are thought to be the result of increased blood flow, improved venous return and improved muscle oxygenation. There is also evidence that muscle jarring and vibration are decreased by compression stockings.

HISTORY OF VARICOSE VEINS
First described by Hippocrates in 460 BC, mankind has suffered from varicose veins for over 2000 years. There have been many methods for treating varicose veins since as early as the ancient Egyptians, most of which were quite "experimental." This included "vein stripping" which is largely no longer performed.

In the 1950s and 1960s, 30% of hospital beds in Europe were taken up by patients admitted for the management of varicose veins or all types. Now it is very rare to see a patient admitted to hospital for this problem.

True or False: Men are not at risk for developing varicose veins and CVI.
False. Men are at risk for experiencing varicose veins as well as more severe signs and symptoms of CVI. In fact, 43% of men are expected to develop venous disease by the time they reach their 60s; however, a majority of those do not seek treatment until the condition worsens.

True or False: Varicose veins and CVI treatments are just too expensive to consider.
False. Because varicose veins and CVI are recognized as clinical conditions, most insurance plans will cover treatment.

True or False: Compression stockings and elevation of the feet are cures for varicose veins.
False. Although compression stockings provide graduated compression from the ankle up to the knee or thigh, which helps to reduce pain and swelling, they only help to manage symptoms and do not provide a cure. In fact, people with varicose veins may need to continue to wear these stockings for the rest of their lives. Likewise, elevating the feet above the thighs when sitting and above the heart when lying down can help to ease pressure on the veins, but elevation, like the use of compression stockings, is not a cure for varicose veins.

True. Those spending the entire day on their feet are at higher risk for developing varicose veins. It is recommended that individuals with careers that require prolonged standing, such as restaurant servers or flight attendants, rest each day by elevating their feet above their heart.

Answer: Weight. Being overweight often contributes to the formation of varicose veins, with the added pressure on the legs and ankles causing the veins to bulge and blood flow to be disrupted. Regular exercise is recommended to increase blood flow in the legs and maintain a healthy weight to mitigate signs and symptoms that can lead to CVI, particularly varicose veins.

Multiple Choice: Which of the following is not shown to contribute to varicose veins and CVI: prior trauma or injury, insufficient hydration or pregnancy?
Answer: Insufficient hydration. Insufficient hydration should not affect the development of varicose veins. However, trauma or injury to the lower limbs can contribute to signs and symptoms leading to CVI, including varicose veins.
