What do I do if I strain my knee at a volleyball match…
So you’re not 20 and you’ve decided to play volleyball with your friends a couple of days per week after work. Everything is going great until you and an older gentleman collide under the net and you feel your knee pop violently. You’re down on the ground, adrenaline pumping, thinking that your ACL may have just been sacrificed to the Recreation League Volleyball Gods.
Assuming that you have pain with ROM and weight bearing, it is a very good idea to head to your local urgent care or ER to rule out major pathology such as a fracture. At this point, an MRI is not typically ordered because of swelling.
When you leave the ER, you may feel like you have more questions than answers. You have just started down a relatively long path towards resolving this issue. Think of this process as a path that has many different stops. Our 21st century brain wants all issues resolved immediately, but patients should recognize that the best course of action is to allow the body to heal and to facilitate that process as best we can. The best thing to do is go to physical therapy within the first week.
Many people think of physical therapy exercises as strengthening exercises, as if it is personal training with some massage therapy. A good orthopedic physical therapist has the ability to determine tissue in lesion using subjective history, objective findings, special tests and facilitate the healing process through a graded exercise program that will generally start with passive range of motion to tolerance and decreased weight bearing as needed.
Probably the most important things that you can do is keep the knee moving through available ROM, decrease weight bearing to tolerance, and allow the knee to heal over the next three to 6 weeks. As the knee calms down, it is important to make sure that the hip is moving correctly and the hamstring/calf are moving well. Then as the knee tolerates more activity, loading can begin.
The real decision making time is at the 6-8 week mark. If the knee is progressing well, and tolerating activities like stairs and squatting, then we start doing some running and light cutting activities. If those go well, then a return to sport program is initiated. There is no need for further investigation.
If the knee continues to swell on a regular basis, if ROM is not full and pain free, and especially if there is an unstable feeling, then an MRI is warranted to rule in or out a ligamentous/meniscal injury. If there are findings on imaging and the patient is appropriate for surgery medically and can benefit due to activity desires, then it makes sense to have surgery. But, also note that by this time hips and pelvis are moving well, hamstring and quadriceps have come back to a certain extent, and the patient is now really ready to have that major surgery.
So often patients want immediate results and immediate action when in actuality, the best course of action likely has a component of procrastination.
