2023 ANNUAL STATEMENTS FOR THE HEALTH AND BENEFIT FUND

The 2023 1095B form will be mailed to all participants starting 2/20/24. If you have an email on file, you should have received an email today from ONLINE FORM RETRIEVAL with the subject: IMPORTANT TAX RETURN DOCUMENT AVAILABLE. The email will contain a link to your 1095B form where you can download or print the form. (PLEASE BE SURE TO CHECK YOUR SPAM OR JUNK FOLDER FOR THIS EMAIL). If you have any questions about THE 1095B form, or any other year-end tax form, please call (212) 331-1800.

Contact the Health and Benefit Fund’s Member Services department with any questions you may have regarding the W-2 for Loss of Time, 1099-R, or 1095-B forms.

2023 ANNUAL STATEMENTS FOR THE HEALTH AND BENEFIT FUND

The 2023 W-2 forms for Sick Fund and for the Health and Benefit Fund Loss of Time will be mailed to all participants by 1/31/24. If you withdrew money from your sick fund account or received loss of time payments in 2023 you should receive a W-2. PLEASE NOTE: You will receive an email from Online Form Retrieval with the subject: IMPORTANT TAX RETURN DOCUMENT AVAILABLE. The email will contain a link to your W-2 form so that you can access the form online. You can follow the link to view and print your W-2 form. If you have any questions about your Sick Fund W-2 please email [email protected] or call (212) 331-1800.

The 2023 1099R for the Death Benefit for the Health and Benefit Fund will be mailed to the paid beneficiary by 1/31/24. The 2023 1095B form will be mailed to all participants by 3/01/24. Contact the Health and Benefit Trust Fund Member Services department with any questions you may have regarding the W-2 for Loss of Time, 1099-R, or 1095-B forms.
